Description
Amendment No. 3 to Request for Quote - Copper Sulfate Aquatic Herbicide for Bureau of Indian Affairs - Flathead Irrigation Project The purpose of Amendment No. 3 is to: 1. Extend the due date for submission of quotes. Quotes are due no later than 4/22/2021 at noon Pacific Daylight Time; Quotes are due by email to Anita.MacAuley@bia.gov. Faxed and mailed quotes will not be accepted. Quotes must include the solicitation number 140A1221Q0010 in the subject line of your email to be considered for award. 2. Clarify how to submit a quote: To be considered for award, you must provide the following: 1. Price (fill out the following information: ____price per 50 lb., Copper Sulfate, Chem One brand, CAS:7758-99-8, Total price_____(Note: the proposed price must be inclusive of freight costs and any hazardous material fees/taxes); 2. Submit a product sheet showing the proposed brand name; if proposing an alternate brand to Chem One, product sheet must show that all salient features are met (see line item detail on form SF 1449 for salient features); 3. Fill out and return the attached document, 52.223-3, Hazardous Material Identification and Safety Data. All other terms and conditions remain unchanged. FAR 52.219-6, Notice of Total Small Business Set-Aside (Nov 2020). (a) Definition. Small business concern, as used in this clause (1) Means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the size standards in this solicitation. (2) Affiliates, as used in paragraph (a)(1) of this clause, means business concerns, one of whom directly or indirectly controls or has the power to control the others, or a third party or parties control or have the power to control the others. In determining whether affiliation exists, consideration is given to all appropriate factors including common ownership, common management, and contractual relationships. SBA determines affiliation based on the factors set forth at 13 CFR 121.103. (b) Applicability. This clause applies only to- (1) Contracts that have been totally set aside for small business concerns; and (2) Orders set aside for small business concerns under multiple-award contracts as described in 8.405-5 and 16.505(b)(2)(i)(F). (c) General. (1) Offers are solicited only from small business concerns. Offers received from concerns that are not small business concerns shall be considered nonresponsive and will be rejected. (2) Any award resulting from this solicitation will be made to a small business concern. (End of clause) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All responsible sources may submit a bid, proposal, or quotation which shall be considered by the agency. The anticipated award date is on or around April 23, 2021. No Defense Priorities and Allocations System (DPAS) assigned rating applies to this solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-03 dated 2-16-2021. Description of requirements for the items to be acquired: Copper Sulfate, Chem One brand, CAS:7758-99-8, qty. 150, 50 lb. bags, for weed control in irrigation canal applications, or equivalent brand. Equivalent salient features: large crystals, active Copper Sulfate Pentahydrate 99%, 50 lb. bag. Below is a list of the line item numbers, and items, quantities, and units of measure: ____price per 50 lb., Copper Sulfate, Chem One brand, CAS:7758-99-8, Total price_____ (Note: the proposed price must be inclusive of freight costs and any hazardous material fees/taxes) Date(s) and place(s) of delivery and acceptance and FOB point: May 14, 2020, Saint Ignatius, Montana The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. You are required to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. FAR 52.223-3, Hazardous Material Identification and Safety Data, applies to this acquisition. FAR clause 52.211-6, Brand Name or Equal (Aug 1999) applies to all line items in this solicitation. See the text of the applicable clauses, attached. How your quote will be evaluated: Award shall be made to the responsible firm submitting the lowest priced, technically acceptable quote. If quotes cannot be determined reasonably priced, no award shall be made. In order for your quote to be determined technically acceptable, you must supply product sheet showing the brand name proposed and if submitting an alternate brand to Chem One, the product sheet must show that all salient features are met. In addition, hazardous material clause 52.223-3 must be properly filled out and submitted with your quote.
