Description

3 4/19/2021 Statement of Work Syracuse, New York VAMC Hot Water Boiler Maintenance, Testing, Repair, and Inspection Background: The Department of Veteran s Affairs, Syracuse VAMC located at: 800 Irving Avenue Syracuse, New York 13210 and The Department of Veterans Donald J. Mitchell Outpatient clinic located at: 125 Brookley Road, Building 510, Rome, NY 13441 is currently seeking recurring preventative maintenance, recurring inspections, technical support, and emergency repair by a Fulton Boiler factory trained and authorized service company on five (5) Fulton Boiler VTG-4000DF s and one (1) Fulton Boiler VTG-4000 condensing hydronic boilers and ModSync boiler controller. All inspections, testing, preventative maintenance, and repair of aforementioned equipment shall be conducted in accordance with Fulton Installation and Operation Manual, Vantage (VTG) Condensing Hydronic Boilers, Standard, Dual Fuel, and Low Emissions Models 2,000,000- 6,000,000 BTU/ HR - VTG-IOM-2018-0828 and the most up to date version of the OCAMES: Healthcare Engineering Standard ES-2018-001 Low Pressure Steam and Hot Water Boilers: Design and Operation. Hours of Work: Normal business hours, 8:00 am 4:30 pm Monday Friday (excluding Federal Holidays). The following are Federal Holidays observed by the facility and the Contractor shall not expect access to the VA Facility of VA employees on these dates unless notified otherwise by the COR. Preventive maintenance or other services are not to be scheduled on these dates: New Year's Day Birthday of Martin Luther King, Jr. Washington's Birthday Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day The Contractor shall notify the Contracting Officers Representative (COR) a minimum of five (5) working days prior to visiting the hospital for inspection purposes. Period of Performance Base Year: 06-01-2021 to 05-31-2022 Option Year#1: 06-01-2022 to 05-31-2023 Option Year#2: 06-01-2023 to 05-31-2024 Option Year#3: 06-01-2024 to 05-31-2025 Option Year#4: 06-01-2025 to 05-31-2026 Inspection and testing frequency shall be dictated by and follow the most up to date version of the OCAMES: Healthcare Engineering Standard ES-2018-001 Low Pressure Steam and Hot Water Boilers: Design and Operation. All Monthly testing listed within this standard shall be completed by VA staff and not required as part of this contract. All other testing listed within this standard shall be completed and properly documented by the contractor. Emergency repair service (site specific): Rome, New York Donald J. Mitchell Outpatient Clinic: Response within 24 hours of notification of any boiler failure or controller malfunction. Syracuse VAMC: 72 hours of notification of any boiler failure or controller malfunction.
