THIS SPECIAL NOTICE IS NOT A REQUEST FOR QUOTES. This notice is for informational purposes only. The intent of this Sources Sought Notice 36C24421Q0538 is to identify any Small Businesses, mainly SDVOSB/VOSB vendors who can provide Patient Monitoring System as listed in the attached Statement of Need. In addition, we are also seeking any Small Business manufacturers that have the capability of providing this system as well. This is neither a solicitation nor a promise of future work. Small Business, SDVOSB, and/or VOSB manufacturers authorized distributors are encouraged to submit your company s information and capability statement in response to this Sources Sought Notice. This notice does not constitute a formal Request for Quote (RFQ), nor is the government obligated to issues an RFQ. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey. Potential vendors should respond with their ability to provide the following brand name: The Department of Veterans Affairs (VA), VISN 4, Network Contracting Office 4 (NCO 4), is seeking information and potential qualified sources capable of meeting the Statement of Work (SOW) for a Brand Name Patient Monitoring System, for Lebanon Veteran Affairs Medical Center (LVAMC), Lebanon, PA. 1700 S. Lincoln Ave., Lebanon, PA 17042 The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of interested parties to demonstrate to the government that they can provide the equipment or supply that fulfills the required minimum specifications mentioned above. DEPARTMENT OF VETERAN AFFAIRS STATEMENT OF WORK Telemetry Expansion on Medical/Surgical Unit GENERAL INFORMATION Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The Lebanon Veteran Affairs Medical Center (LVAMC), Lebanon, PA, has a need to replace spacelabs medical equipment for its PACU/OR, ICU and Medical-Surgical units due to end of life and/or unsupported technology. Lebanon VAMCs infrastructure is such that the requirement must be for equipment/materials that are supportable and equal to systems that are currently in place. Background: The Lebanon VAMC provides outpatient and emergent medical procedures through its Perioperative Care and inpatient care via Intensive Care Unit and Medical-Surgical Unit. The capability of having lightweight, transportable and efficient system flow is essential for the highest quality patient monitoring and medical management. Equipment must have the same functionality throughout the different locations to allow for staff familiarity with processes and enhanced safety. This is a Brand Name Spacelabs 91393 Xprezzon (Space Labs Healthcare) requirement. B. GENERAL REQUIREMENTS The contractor shall provide all equipment, tools, materials, documentation, services, and other items necessary to deliver, assemble, and ensure proper functioning of all patient monitoring/medical management systems purchased. Patient Monitoring System: The facility requires replacement of its patient monitoring system in its perioperative area and inpatient units. The patient monitoring system requires the following specifications/features at a minimum: Characteristics Lightweight Withstand accidental drops Functions at bedside or during transport IV pole mount Touchscreen display with alarm Remote viewing Wireless networking/integrated battery supports no less than 5 hours Stores up to 96 hours of trends Easily moved between monitors Transfer up to 24 hours of patient data between monitors without having to re-cable the patient. Refer to Attachment A for more details Deliverables include: Supplies and Accessories, Support infrastructure (cabling, connectors, etc.), Project Management, Installation and Training. Quantity Description Model # 24 Xprezzon Monitors 91393 2 Qube Compact Monitors 91390 8 Xprezzons (ED) 91393 2 Qube Mini Transport Monitors 91389 11 Xprezzons 91393 3 Command Modules 91496 INSTALLATION: The Contractor shall provide a system Installation Plan (IP) and sample schedule to the Contracting Officer Representative (COR) with proposal. The Installation Plan shall include estimated timeline of delivery, system calibration, and training. Perform staging, programming, and configuration Perform all necessary test/audits TEST AND ACCEPTANCE: Contractor shall test all equipment after assembly/configuration. WARRANTY/SERVICE CONTRACT: All equipment and materials shall come with a standard one-year warranty. Warranty shall begin after completion of tests. GENERAL RESPONSIBILITIES: Manage all aspects of the equipment, technology, clinical workflow, and go-live activities from the initial kickoff to project closure. Training: two sessions of 3 consecutive weekdays of customized clinical training. Up to 7 hours of class time per day, between the hours of 0800 and 2000. Maintaining environmentally safe environment per hospital regulations/codes Disposal of old equipment/materials Upon completion, work areas will be without damage as prior to work commencing. D. OTHER CONSIDERATIONS: CONTRACTING OFFICER REPRESENTATIVE (COR): The Contracting Officer Representative or designated Biomedical personnel shall be the technical point of contact for all work-related requirements. The COR does not have authorization to change, alter, or remove any requirements stated in the performance work statement. The Contracting Officer is the only government authorized person to change the performance work statement or any resultant contract. PLACE OF PERFORMANCE: Lebanon VA Medical Center. WORK HOURS: Standard Operational Hours are 0700-1600, Monday-Friday DELIVERY: Delivery shall be within 30 days of contract award All deliveries shall be coordinated with the designated Biomedical personnel and COR before arrival. Patient monitoring system to be delivered to the Warehouse at the Lebanon VA Medical Center, 1700 S. Lincoln Ave., Lebanon, PA 17042. Responses to this Sources Sought should include company name, address, point of contact, phone number, point of contact e-mail, DUNS Number, Cage Code, size of business pursuant to North American Industrial Classification System (NAICS) 334220, (size standard of 1250 employees). Respondents shall also answer the following questions: Size status and representations of your business, such as but not limited to: Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), HUBZone, Woman Owned Small Business (WOSB), Small Business (SB), Large Business, etc. Is your company considered small under the NAICS code identified under this Source Sought? Is your company the manufacturer, distributor, or equivalent source for the type of items required with the characteristics referenced above? If you accompany is a large business, do you have any authorized distributors? If so, please provide the name(s), telephone Number(s), Point(s) of Contact and size status of the businesses authorized to distribute your products. If your company is a small business and an authorized distributor/reseller for the types of items required with the characteristics referenced above, do you alter; assemble; or modify; the items requested in any way? If your company alters the items, state how and what is altered; assembled; and/or modified? Does your company have an FSS contract with GSA or the VA NAC or a GWAC contract holder with any other federal agency? If so, please provide the contract number. If your company holds a FSS GSA/VA NAC contract other federal GWAC contract holder, are the items you are providing information on available on your schedule/contract? General pricing of your products is encouraged. Pricing will be used for the purpose of market research only. It will not be used to evaluate for any type of award. Information provided by potential sources will be utilized by the Government to determine market availability and procurement strategy. An award of a contract will not result from this Sources Sought Notice. Vendors who can meet the requirements listed above should provide the following information: Business Name Business Address Business Point of Contact, including phone number and email address Business size Business Socioeconomic Status Federal Supply Scheduling (FSS) number, if the items required by the Government are available for purchase on an FSS contract. Any other pertinent capability information that relates to the above stated requirement.
