Request for Information The government is looking for interested and capable contractors to complete the requirement described here-in. Please see the Performance Work Statement below for additional details from page 1 through 2. All interested vendors should provide a capability statement and previous contract information, if awarded by the VAMC for a similar requirement. Jones-Banks / Contracting Officer Dionna.JonesBanks@va.gov Performance Work Statement Contract Title: Columbus VAMC: Integrated Dual Diagnosis Treatment (IDDT) Services Background: Since October 2012, the VA Medical Centers (VAMCs) have been acquiring non-personnel services. These services are inclusive of Integrated Dual Disorder Treatment (IDDT) and Assertive Community Treatment (ACT) Evaluations and Training. Currently the Columbus VAMC, Chalmers P. Wylie Ambulatory Care Center needs a vendor who is capable of providing the services mentioned above and help with sustainment of implementation efforts made at this facility. Scope: The Columbus VAMC is committed to advancing Veterans Healthcare and benefits. Provide IDDT and ACT evaluation and training to Chalmers P. Wylie Ambulatory Care Center would like to procure the following services: Integrated Dual Disorder Treatment (IDDT) Evaluations and Training Assertive Community Treatment (ACT) Evaluations and Training Evaluation and Training for the Intensive Community Mental Health Recovery (ICMHR) Services programs. The contractor shall provide all personnel, equipment, supplies (except those supplied), facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary perform the IDDT and ACT requirement as defined in this PWS, except for those items specified as government furnished property and services. Period of Performance: The period of performance shall be for one (1) Base Year and four (4) 12-month option years summarized as follows: Base Year: TBD based on contract award, tentatively, 05/01/2021 04/30/2022 Option Year One (1) TBD Option Year Two (2) TBD Option Year Three (3) TBD Option Year Four (4) TBD .
