- This is a Combined Synopsis/Solicitation is for a SKYPLATE Detector Replacement, Philadelphia VA Medical Center, as prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated. (ii) The solicitation number is 36C24421Q0589 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2021-05 dated 3/10/2021. (iv) This solicitation is set aside 100% Total Small Business Set-Aside and the associated NAICS code 334516 has a small business size standard of 1000. (IV) Schedule of Requirement/CLINS (v) Statement of Work/Salient Characteristics Statement of Work: BRAND NAME - PHILIPS SKYPLATE DETECTOR (Replacement) ___________________________________________________________________________ STATEMENT OF WORK The Department of Veterans Affairs, Philadelphia VA Medical Center, has a Brand Name requirement for a SKYPLATE Detector. BACKGROUND: CMC VAMC Diagnostic Radiology Department has a bona fide requirement to procure a replacement for a Philip SKYPLATE Detector that will be in conjunction with the hospital s portable x-ray machine. The current detector is considered inoperable due to improper handling by the hospital s staff. The SKYPLATE Detector is an imaging receptor that is key for the functioning of the facility s Philips Healthcare X-Ray machine. This equipment part is mandatory, because the portable x-ray machine cannot take an image without it in place. The equipment is designed to provide high-resolution imaging, so that x-rays can be taken that achieve (DQE) Detective Quantum Efficiency. SCOPE OF WORK: This SOW covers the following activities and deliverables which includes the manufacture and delivery of one replacement SKYPLATE Detector to be used with the Diagnostic Radiology Department s portable X-Ray scanner. ITEM NO. PART NO. Ability to transmit images remotely to the Philips iSite/Intellispace system. Lightweight and designed for high-resolution imaging. Must be new. Must include rechargeable battery. Training and Installation: Training and installation not required by Vendor. Manufacture Distribution Letter Required: Yes Delivery: All items must be FOB Destination, 30 Days ARO, to the following location: Philadelphia VA Medical Center 3900 Woodland Avenue Philadelphia, PA 19104 Monday Friday from 8:00am 4:00pm (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and the following clauses AND instructions are added as addenda: 52.252-1 Solicitation Provision Incorporated by Reference (FEB 1998) This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufacturers or gray market items will be acceptable. Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty, and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
