Description
Amendment 001 is provided to extend the quote due date, update Attachments 1-4, and add Attachment 9.� Please see attaachments for details. This notice is a combined synopsis/solicitation for commercial items/services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information attached to this notice.� This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The Government intends to acquire a commercial item using FAR Part 12 and the Simplified Acquisition Procedures set forth in FAR Part 13. This notice is being issued as a Request for Quote (RFQ) under solicitation number 80KSC021R0027 for National Aeronautics and Space Administration (NASA) Employee International Medical Support for NASA civil service employees traveling outside the Continental United States on official government business, as described in the attached Statement of Work (SOW). �Services to NASA travelers will be provided 24/7/365 on a firm-fixed-price basis and classified as CLIN 001.� Support would be provided telephonically and/or electronically to include assisting travelers with finding local pharmacies; arranging for replacement or repair of medical devices (e.g., eyeglasses, mobility devices); identifying for travelers where to obtain non-emergency medical/dental assistance, and coordinating with appropriate offices to assist travelers with lost passports.� If required, additional services that would require pre-coordination and communication with NASA-designated points of contact include emergency ground (e.g., ambulance), air transportation services, hospital/medical facility (for stabilizing a patient) arrangements, medical/surgical procedures planned, medications to be used, specialty medical professionals participating in patient care, and repatriation of mortal remains, which would be invoiced in detail to NASA on a cost-reimbursement basis if/when expenses are incurred. �These expenses would be classified as CLIN 002. The Period of Performance of this purchase order will be as follows: ���� Base Period:������������� �07/01/2021 to 09/30/2021 ���� Option Period 1:�������� 10/01/2021 to 09/30/2022 ���� Option Period 2:�������� 10/01/2022 to 09/30/2023 ���� Option Period 3:�������� 10/01/2023 to 09/30/2024 ���� Option Period 4:�������� 10/01/2024 to 09/30/2025 ���� Option Period 5:�������� 10/01/2025 to 06/30/2026 The NAICS Code and Size Standard are 624230 Emergency and Other Relief Services and $35M, respectively.� The offeror shall state in their offer their size status for this acquisition. All responsible sources may submit an offer which shall be considered by the agency. This posting, in addition to any attached documents, will be available on www.beta.sam.gov. �Potential offerors will be responsible for downloading their own copy of this combination synopsis/solicitation and amendments (if any). Prospective offerors shall notify this office of their intent to submit an offer.� All contractual and technical questions must be submitted electronically via email to Jennifer Forhy, jennifer.d.forhy@nasa.gov not later than 12:00pm Eastern Daylight Time on Friday, April 16, 2021.� Telephone questions will not be accepted.
