- Page 3 of 3 Sources Sought Notice 1.Title: NEW SELLICK ROUGH TERRAIN FORKLIFT 2.Purpose: This Sources Sought/Market Survey Notice is being conducted by the Department of Veterans Affairs, Network Contracting Office (NCO) 16 in support of the Central Arkansas Veterans Health Care System (CAVHS) located in North Little Rock, Arkansas to identify other sources that are capable of providing NEW Sellick Rough Terrain Forklift. 3.Objective: To find interested vendors with the capability to provide NEW Sellick Rough Terrain Forklift as listed in the equipment schedule below: Equipment Schedule: Manufacturer Part No. Description: Sellick S80J4e-4 Rough Terrain Forklift: 8,000lb Base Capacity 74hp Tier 4 Diesel (T4F) 4WD 4. The North American Industry Classification (NAICS) code for this acquisition is 333924- Industrial Truck, Tractor, Trailer and Stacker Machinery Manufacturing. Standard in number of employees is 750. 5. All questions shall be answered in Section 7 for the government considered contractor with the capability to meet the government requirement. 6. Deliver to: Central Arkansas Veterans Health Care System Warehouse, Building 182 4300 W. 7th Street North Little Rock, Arkansas 72205 THIS IS NOT A REQUEST FOR COMPETITIVE QUOTES/PROPOSALS AND NO SOLICIATION IS AVAILABLE. 7. Responses Required: The CAVHS requires all the following questions answered in this sources sought notice. Answers that are not provided shall be considered non-responsive to the Sources Sought/Market Survey Notice and Contractor shall not be considered as part of the market research. Telephone requests will not be honored. No award will be made based on quotes or offers received in response to this notice. All responsible sources may identify their interest to meet this requirement to Jeneice McClinton, jeneice.mcclinton@va.gov by 12:00 pm (Central Time) on April 28, 2021. Additionally, any response to this notice must show clear, compelling and convincing evidence that competition will be advantageous to the government by providing the following: 7.1. Business Size and Socio-Economic Status: (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a Vendor Information Pages (VIP) verified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. (h) State whether your firm is registered with the System for Award Management (SAM) at www.sam.gov and/or the VetBiz Registry at www.vetbiz.gov . If not, please NOTE: any future solicitation could only be awarded to a contractor who is registered in SAM. To receive credit as a SDVOSB or VOSB status you must be registered in the VetBiz Registry. 7.2 Are you the manufacturer? 7.3 Can you meet the non-manufacturer s rule? 7.4 Do you have an existing Blanket Purchase Agreement (BPA), General Services Administration Schedules (GSA) or Veteran Affairs (VA) Federal Supply Schedule (FSS) contract? If yes, please include the contract details and a brief summary of the products and services provided in your response. 8. Statement of Capability: Provide a capabilities statement/ short summary of your potential approach to this type of contract and meeting the specific requirements per the sources sought notice and objectives and your experience managing similar contracts with similar requirements for the CAVHS. 9. Instructions and Response Guidelines: Sources sought responses are due by April 28, 2021 at 12:00pm (Central Standard Time); size is limited to 8.5 x 11 inches, 12-point font, with 1-inch margins in Microsoft Word or PDF format via email to jeneice.mcclinton@va.gov .
