Attachment 1: Combined Synopsis-Solicitation for Commercial Items VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Original Date: 10/12/17 Revision 04 Date: 06/10/20 Description (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. (ii) This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05. (iii) This solicitation is full and open competition in the open market pursuant to FAR 13.104. The contract type for this procurement is single award, firm-fixed price, 1-year base plus four 1-year options. (iv) The associated North American Industrial Classification System (NAICS) code for this procurement is 334516 Analytical Laboratory Instrument Manufacturing, with a small business size standard of 1,000 employees. The FSC/PSC is 6630 Chemical Analysis Instruments. (v) The Department of Veterans Affairs, VHA Regional Procurement Office West, Network Contracting Office 22 in Long Beach California is seeking to purchase CyFlow type antibody reagents. It will be used as part of a lab-developed test for flow cytometric analysis of leukemias and lymphomas. (vi) All interested companies shall provide quotations for all items listed on Attachment A, otherwise it will be considered non-responsive. See Attachment A for more details. 1) On Attachment A, please provide extended price for all line items. 2) Please provide extended price for all line items for base year and four 1-year option periods. (vii) The items will be ordered on an As needed basis. The Government will issue a single award contract and obligation number to the responsible quoter whose quotation conforming to the solicitation will be most advantageous to the Government, price and other decision factors considered. (xii) The following are the decision factors: 1. Price: The Government will evaluate the price by adding the total of all line item prices, including all options. The quoter s total evaluated price for the purpose of evaluation will include the base period and all option periods. The total evaluated price will be that sum. 2. Past Performance. The past performance evaluation will assess the relative risks associated with a quoter s likelihood of success in fulfilling the solicitation s requirements as indicated by the quoters record of past performance. (xiii) Evaluation Approach. The Government will evaluate quotations/offers using the comparative evaluation process outlined in FAR 13.106-2 (b)(4), where quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). The Government will determine price reasonableness based on competitive quotations/offers received. Quotations/offers will be evaluated side-by-side comparison of vendors proposed prices. (xiv) All quoters shall submit only one quotation/offer. The Government is not requesting or accepting alternate quotations/offers. (xv) To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" OR ""The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"" Quoters shall list exception(s) and rationale for the exception(s), if any. Responses should contain your best terms and conditions. (xvi) All offeror quotes for this solicitation must be received electronically by email no later than Friday, May 7, 2021, 4:30 p.m. PST. Any offeror quotes received after the prescribed date and time shall be considered non-responsive and will not be considered for award.
