Description
The purpose for this source sought notice is to locate interested companies that can furnish the REQUIREMENTS listed in the SOW that is attached. Offeror's capability to meet this requirement including any current contracts with more than 2 years of Period of Performance (Civilian or Government) to include meeting the below listed accreditations or similar standards, facility description, capabilities, and certifications. Responses must show clear and convincing evidence that your company and its employees have the capabilities, training, and qualifications to provide this service to be considered as a source. The Government may elect to award a Firm Fixed Price contract resulting from the solicitation. Responses to this market survey should be emailed to shawn.vanzandt@va.gov. No telephone responses will be accepted. Please provide a detailed response to the below Performance Work Statement stating how you can meet all aspects stated. It is requested that responses be received no later than 2:00 pm Central, May 12th, 2021. *NOTE* SDVOSB Contractors to provide documentation to fulfill capabilities of the following clause: FAR 52.219-27. No solicitation document is available currently; this notice is to acquire information only. Vendors interested in providing an offer will have to respond to a separate solicitation announcement. Performance Work Statement 1. Background: 1.1 We are requesting the purchase of a quantity of 7 each Haag-Streit AT 870 Applanation Tonometers or equivalent. We are requesting these units because they are required by the VHSO Eye Clinic to perform patient exams for the Fort Smith CBOC Activation. These instruments are used to measure interocular pressure of the eye. The measurement can be carried out on the patient sitting at the slit lamp in conjunction with other routine microscopic examinations. The accuracy of the measurement is extremely high. The average deviation in any one single examination does not exceed +/- 0.5. mm HG and repeated measuring procedures do not reduce the ocular pressure, as no massing effects occurs, and there are no standardization or calibration difficulties. The AT 870 is designed to swing down into position from above the slit lamp microscope, and it may be removed from the slit lamp if required. In it s parked position the AT 870 does not interfere with use of the slit lamp. These tonometers are used in each Eye Clinic location (FAV, SFD, BRN) and therefore are necessary for standardization. 2. Scope: 2.1 The Contractor will furnish all labor, transportation, parts, and expertise necessary to provide the VHSO with 7 each Exam Applanation Tonometers and the necessary training. 3. Salient Characteristics for Applanation Tonometer: 3.1 Must be able to be installed on or over the slit lamps 3.2 The measuring force of the instrument must be generated via spring force. 3.3 Must have a measurement range between 0-80 mm Hg. 3.4 Must have a backlash width of <0.25 mN. 3.5 Must include a manufacturer s limited warranty. 4. Delivery Schedule: 4.1 The requested equipment is required by 9/30/2021 and will be delivered to the VHSO as soon as possible. 4.2 The Contractor will ensure all deliverables are delivered to the following address and reference the VA s purchase order number as an Attn: line in the shipping address: Veterans Healthcare System of the Ozarks, Attn: Joe Russell, Warehouse Building 8, 1100 N. College Avenue, Fayetteville, AR 72703. 5. Contractor Requirements: 5.1 The Contractor shall not perform any service or provide any item that is outside the scope of work without the written approval of the Contracting Officer. 5.2 Should the Contractor have to come to the VHSO for any reason other than delivery, the Contractor shall be required to log in with VA Police Dispatch (Building 1, first floor) upon arrival to obtain a contractor badge. The Contractor will always visibly wear the badge while on VHSO premises and will be required to turn in the badge before leaving VHSO premises.
