- STATEMENT OF WORK Minneapolis Veteran Affairs Health Care System Annual Fertilizer and Weed Control Service Introduction: The lawn on the MVAHCS campus requires application of fertilizer and weed control in order to establish and maintain a healthy lawn. This service must be performed three times a year. The application area is approximately 25 acres. Objectives: Contractor will provide three applications of fertilizer and weed control products each year based on the season. These products will maximize the growth of a healthy lawn and provide exceptional weed control. This will be a multi-year contract with a base year period of performance of 05/01/2021-04/30/2022 and 4 option years. One on site assessment is authorized in order to evaluate the work to be performed and provide the most accurate price quote. Assessment must be completed prior to the closing date of the solicitation. Coordination will be through the Contract Specialist via email at Cameron.rick@va.gov. Statement of Work: Test soil conditions to determine proper fertilizer and weed control products are utilized Contractor will apply three lawn treatments throughout each year at proper intervals to maximize healthy lawn and weed control prevention: Early Spring Apply seasonal fertilizer and weed control products Early Summer Apply seasonal fertilizer and weed control products Early Fall Apply seasonal fertilizer and weed control products Granular weed control products must be used. Products used must conform to the USDA BioPreferred Program: The Contractor must comply with Sections 9002 of the Farm Security and Rural Investment Act of 2002; the Food Conservation and Energy Act of 2008; and the Agricultural Act of 2014; and the Federal Acquisition Regulation to provide biobased products. The Contractor shall also comply with the clause at FAR 52.223-2, Affirmative Procurement of Biobased Products Under Service and Construction Contracts contained in Section I. The Contractor shall also comply with the provision at FAR 52.223-1, Biobased Product Certification contained in Section K. The Contractor shall utilize products and material made from biobased materials. All supplies and materials shall be of a type and quality that conform to applicable Federal specifications and standards. All supplies and materials to be used in the performance of work described herein are subject to the approval of the Contracting Officer Representative (COR). Biobased products that are designated for preferential procurement under USDA s BioPreferred Program must meet the required minimum biobased content as stated in USDA s Final Rules available at www.biopreferred.gov. The Contractor shall provide data for their biobased products such as biobased content. In addition to the biobased products designated by the USDA BioPreferred Program, the Contractor is encouraged to use other commercially available biobased products such as biobased products certified with a label. The Contractor shall submit with the initial proposal a complete list of biobased products, indicating the name of the manufacturer, cost of each material, and the intended use of each of the materials that are to be used in carrying out the requirements of this contract. Within thirty (30) days of contract award, the Contractor shall provide a complete list of USDA designated biobased products that it intends to use or supply during performance of this contract, including products used or supplied by subcontractors. For each product, the Contractor shall identify the product and brand/manufacturer. For each designated product, the Contractor shall also identify the biobased content. The list must be reviewed and updated annually, and as required by the COR. Reporting is required by Sections 9002 of the Food Conservation and Energy Act of 2008 and the Agricultural Act of 2014. The Contractor shall provide quantities, product types and dollar value of any USDA-designated biobased product purchased by the Contractor. This important data shall be submitted to the COR for the current quarter and current contract year. A brief explanation shall include the methodology used to calculate the dollar value of USDA-designated products used or supplied. Each Product will be applied in accordance with product s manufactures recommendations. Project Deducts: None. Asbestos or Lead: Not applicable Working hours: Anticipated hours of work will be Monday through Friday from 7:am 5:00 pm. Additional hours must be coordinated through the COR. Work may also be performed on weekends or federal holidays with approval from COR but will not affect pricing of the contract. MSDS References: none Safety: Contractor is responsible for maintaining a safe working environment for their employees as well as the MVAHACS Staff. Work areas will be kept cleaned throughout the day and all trash and demolished materials properly disposed of daily. Appropriate Personnel Protective Equipment will be worn while performing work on the VA Campus. Security: Contract personnel need to obtain a vendor s badge form VA Security located at the outpatient entrance security desk. A valid government issued form of identification will be required to receive a badge. A charge of $25 will be assessed for any badges that are not returned at the completion of the project Contractor s Technicians are required to check in with the COR or Grounds Supervisor upon arrival at facility to obtain access and any additional instructions, task explanations and project location information. Daily performance log sheets are required to be submitted to the CO and COR daily. Parking/Storage: Parking is available in parking lots 11, 43A and 43B for vehicles (automobiles and Pickup trucks). Larger vehicles, equipment, job site trailers or storage containers must utilize parking lot 43A only. Contractor s contact information (name and phone number) is required to be displayed on all vehicles, equipment, trailers or containers that are stored overnight at the MVAHCS. Dropping off tools and materials at the loading dock is permitted but no parking is allowed in the loading dock area. Unattended vehicles in unauthorized parking areas will be ticketed by MVAHCS Police.
