- 1. This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. 1.A. Project Title: Air Handler Cleaning Services 1.B. Description:� The HVAC System in the Canteen Service needs to have the air handler, supply, and return thoroughly cleaned. 1.C. Project Location: VA Long Beach Healthcare System located at 5901 East 7th Street, Long Beach, CA, 90822 � Building 165, Canteen Service. 1.D. Solicitation Type - Number: Request for Quotes (RFQ) - 36C26221Q0667 1.E. Set Aside: This solicitation is 100% set aside for Service-Disabled Veteran-Owned Small Business entities. 1.F. Applicable NAICS code: 238220 Plumbing, Heating and Air Conditioning Contractors. 1.G. Small Business Size Standard: $16.5 Million 1.H. Type of Contract: Firm Fixed Price 1.I. Period of Performance: 45 days 1.J. Wage Determination: SCA Wage Determination No. WD 2015-5613, revision 16 2. Scope of Work: The VA Long Beach Healthcare System is seeking a contractor to provide services for air handler cleaning. 2.A. Specific Requirements. 2.A.1.� Contractor shall provide all Labor, Parts, Materials, Equipment, and Supervision required to perform all work. 2.A.2 Contractor is required to have completed the Occupational Safety and Health Administration (OSHA) 10-hour Construction Safety training and 30-hours Construction Safety courses. 2.A.3.� Contractor shall check-in with the Contracting Officer�s Representative (COR) upon arrival at the VA site. 2.A.4.� Contractor and Employees are required to wear Temporary ID Badges that are issued from the VHA Police Department while on VHA property. 2.A.5.� Contractor will always be required to wear all Personal Protective Equipment (PPE). 2.A.6.� Collect, contain, and dispose of all debris resulting from the overall scope of the project according to VHA Regulations. 2.A.7. Contractor shall thoroughly clean the direct and indirect areas where work was performed. 2.B.� Contractor will coordinate with Contract Officer�s Representative (COR) and/or Shop personnel prior to work beginning and daily thereafter. 2.B.1.� Contractor shall thoroughly clean the direct and indirect areas where work was performed. 2.B.2.� Upon completion contractor will contact the Contract Officer�s Representative to inform of the completion of service. 2.B.3. The Contractor shall perform an Activity Hazard Analysis (AHA) and submit the AHA to the Contracting Office Representative prior to any work being performed. The AHA shall be submitted at least three (3) business days prior to the scheduled date that work shall commence 2.B.4. Perform all work after hours, Mon-Fri, between 5pm-11pm. 2.B.5. Contractor will remove all visible debris and containment from the HVAC System and properly dispose. 2.B.6. Contractor will complete cleaning of the AHU #4 and all associated horizontal, and vertical supply, all returns and main ducts and branch ductwork. 2.B.7. Contractor will complete cleaning of the 8 REC heat coils and all supply and return diffusers. 2.B.8.Contractor shall use a HEPA filtered vacuum. 2.B.9. Contractor shall apply DP101 grey sealant to duct system showing leaks and debris. � 2.B.10. Contractor shall comply with current National Air Duct Cleaners Association (NADCA). 2.B.11. The project shall be supervised by a NADCA Certified Air System Cleaning Specialist Contract Modifications 3.A.1. Modifications for this project must be submitted to the assigned COR who will then submit a formal request to the Contracting Officer (CO). New work is not allowed to commence prior to Contracting Officer�s signed and written approval via fully executed Standard Form (SF) 30 modification. 3.A.2. The Modification is considered official only when the Contracting Officer and Contractor sign the Modification and the CO provides all parties a copy of the contract Modification. 3.A.3. Identification, Parking Smoking and VHA Regulations 3.A.4. Contractor shall obtain a �Contractor I.D. Badge� from the VA Police. All Contractor personnel shall ALWAYS wear required visible identification issued by the VA Police during the entire time they are on the VA Healthcare System campus. 3.A.5. The COR is responsible for determining the type of Temporary ID badge that will be issued to the Contractor (including Sub-Contractors). All ID badges will be issues via the VHA Police. 3.A.6. Contractors (including Sub-Contractors) are required to wear Temporary ID Badges that is determined by the COR: One to fourteen (1-14) day temporary Fifteen (15) consecutive days or greater.� 3.B Parking information is available from the VA Police & Security Service Office. It is the responsibility of the Contractor to park only in appropriately designated parking areas. The VA shall not validate or make reimbursement for parking violations of the Contractor�s personnel under any circumstances. 3.B.1. Smoking is prohibited throughout the VA Healthcare System campus. 3.B.2. Possession of weapons is prohibited while on the ANY VA Healthcare System campus. Enclosed containers, including tool kits, shall be subject to search / seizure. 3.B.3. Violations of VA regulations may result in citation answerable in the United States Federal District Court, not at/in the local district, municipal, or state court. 3.B.4. Government-Furnished Equipment / Government-Furnished Information:� No government equipment will be used by the awarded Contractors. 3.C. Risk Control: 3.C.1. If applicable, the Contracting Officer�s Representative will work with VHA Safety and Infection Control services to establish an Interim Life Safety Measures (ILSM) and Infection Control Risk Assessment (ICRA) that must be followed by the Contractor. 3.C.2 VHA requires documentation of Tuberculosis (TB) risk assessment for contractors, and dependent on the project risk level, documentation of results of TB testing and follow-up of those with positive results. VHA Directive 2011-036. 3.D. Insurance Coverage. 3.D.1 The Contractor agrees to procure and maintain, while the contract is in effect, Workers Compensation and Employee�s Public Liability Insurance in accordance with Federal and State of California and/or Nevada laws. The Contractor shall be responsible for all damage to property, which may be done by him, or any employee engaged in the performance of this contract. 3.D.2. The Government shall be held harmless against any or all loss, cost, damage, claim expense or liability whatsoever, because of accident or injury to persons or property of others occurring in the performance of this contract. 3.E. Before commencing work under this contract, the Contracting Officer shall require the Contractor to furnish certification from his/her insurance company indicating that the coverage specified by FAR 52.228-5 and per FAR Subpart 28.307-2 has been obtained and that it may not be changed or canceled without guaranteed thirty (30) day notice to the Contracting Officer. 3.E.1 Any overtime or holiday pay that may be entitled to the Contractor�s employees shall be the sole responsibility of the Contractor and shall not be billed to nor reimbursed by the Government 3.E.2. If a holiday falls on Sunday, the following Monday shall be observed as the legal holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a legal holiday by U.S. Government agencies.
