- Solicitation Amendment 2 (5 May 21) 1.� The purpose of this amendment (Amendment 2) is to provide the Government's Response to Vendor Questions (attached) and to provide a revised RFP (attached)�w/ one revised attachment (attached). 2.� In addition, the proposal submission date is extended until 20 May 21 at 12 Noon ET (per the amended RFP). ---- End Amendment 2 ---- Solicitation Amendment 1 (21 Apr 21)� 1.� The purpose of this amendment (Amendment 1) is to clarify (sync up language) for the Call Tier Information in the following Solicitation Sections/Attachments: �� - Section B� -� CLIN's 0002, 1002, 2002, 3002, 4002, and 5002. �� - Section J�� -� List of Documents, Exhibits, and Other Attachments �� - Section L� -� Additional Instructions to Offerors �� - Attachment 3 - Call Volume Matrix Pricing Sheet �� - Attachment 12 - Call Volume Pricing Matrix 2.� See amended documents (3) for changes to include Solicitation (Amendment 1) W81XWH21R0033, Solicitation Attachment 3, and Solicitation Attachment 12. ----End Amenement 1---- This Solicitation is for a non-personal services contract to provide a DHA J-9 an�Transition Support Contact Call Center Program (operationalized by the name �inTransition�) to transitioning service members or veterans who are currently receiving psychological health care (i.e, mental health care or behavioral health care) and wish to receive care at their new destination or with a new provider in their same location, or wish to initiate psychological health care for the first time. The Psychological Health Center of Excellence (PHCoE) seeks Contractor support to establish and maintain a 24 hours a day, 7 days a week (24/7) toll-free contact call support center that operates 365 days per year. Additionally, the inTransiton call center will be staffed by professionals who are trained and licensed to provide mental health care to bridge the transitional knowledge gap between identification and receipt of psychlogical health care services for a broad range of users, including Active Duty Service Members, Reserve Component, National Guard, Coast Guard, and Veterans and their family members. Additional tasks are contained in the Performance Work Statement included in this Solicitation (Section C). �All prospective prime contractors are provided an opportunity to submit proposals under this request. Only one award is anticipated resulting from this RFP; however, the Government reserves the right to withdraw, modify, or eliminate various aspects of the requirement determined to be too costly or impractical prior to award.� See attched Solicitation for details and instructions.
