Description: 21 May 2021 MEMORANDUM FOR ALL PROSPECTIVE OFFERORS FROM: USPFO for Ohio Purchasing and Contracting Office �� SUBJECT: �Establishment of Blanket Purchase Agreement (BPA) for CIF Laundry 1.�� The Ohio Army National Guard (OHARNG) proposes to establish a Blanket Purchase Agreement (BPA) to provide CIF Laundry Services as described in the attached bid schedule. A BPA is not a binding contract, they are agreements, and as with this agreements, prices must be set prior to any orders being placed against the BPA. �Additionally, since it is an agreement and not a contract, either party may choice to terminate an agreement provided a 30-day notification is given. � 2.�� In addition to the bid schedule, please find attached general terms and conditions of a BPA. �If you are interested in establishing a BPA with our office, please complete the attached bid schedule of any and all items your company can supply, do not add any items to list. �You are encouraged to submit your lowest prices including any discounts available to the OHARNG. Our intent is to establish at least one, but no more than three BPAs for this service. A BPA awarded based on past performance and pricing, so I cannot stress enough, include your best pricing terms. �Furthermore, pricing for the initial year of this agreement shall not be revised unless authorized by a Contracting Officer. � � 3.�� Return the attached bid schedule with your pricing to my attention later than 12 June 2021 at 10:00 a.m. Any questions are due by 12:00pm on 10 June 2021. If your company is selected for BPA, I will forward you additional documents as part of the BPA award. 4.�� Bid Schedules can be e-mailed to: matthew.s.keen.civ@mail.mil. Should you have additional questions, please do not hesitate to contact me via email. �� Attachments 1.�� Terms and Conditions (IAW FAR 13.303-3) 2.�� Price List 3.�� Item List � W91364-21-A-XXXX�� TERMS AND CONDITIONS (IAW FAR 13.303-3) 1. �This is a �Blanket Purchase Agreement� (BPA) which is a simplified form of a monthly charge account. �If awarded, you will be authorized to provide Laundry Services, in accordance with the attached Schedule and Price List, when called by the Contracting Officer or their designated representative(s) during the period of 1 July 2021 through 31 May 2026.� 2. �It is understood and agreed that this BPA obligates the government only to the extent that authorized calls are actually made against it and then only when delivery against such calls has been completed.� 3. �Prices to the government are in accordance with the price list submitted by the contractor and approved by the contracting officer. �Prices to the government shall be as low or lower than those charged supplier�s most favored customer for comparable quantities under similar terms and conditions, in addition to any discounts for prompt payment.� 4. The Contracting Officer will provide the Contractor a list of individuals authorized to purchase/request service under this BPA identified by organizational component, and the component, and the dollar limitation per purchase for each individual.� 5. �Each delivery will be accompanied by a CALL order which will contain all of the following information (as a minimum): � � � a.�� Name of vendor. b.�� BPA number. c.�� Date of call and call number. d.�� Name of person placing the call. e.�� Itemized list of supplies/services furnished. f.�� Quantity, unit price, and extension of each item, less applicable discount. g.�� Date of delivery. 7. �A summary invoice shall be submitted at least monthly or upon expiration of the BPA, whichever occurs first, for all deliveries made during a billing period, identifying the delivery tickets covered therein, stating their total value, and supported by receipted copies of the delivery tickets. 8. Invoices will be submitted through Wide Area Workflow (WAWF).
