- Page 8 of 8 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05 (eff. 3/10/2021). This solicitation is set-aside for Small Business Concerns. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112, Surgical and Medical Instrument Manufacturing, with a small business size standard of 1.000 employees. The FSC/PSC is 6515, Medical and Surgical Instruments, Equipment, and Supplies. The Phoenix VA Health Care System is seeking to purchase BRAND NAME ONLY Brainlab Kick EM, Electromagnetic Surgical Navigation System manufactured by Olympus America, Inc. for the ENT surgeons. Interested Small Business concerns must have a signed Letter of Authorization from Olympus America, Inc., specifying that their company is an authorized distributor/reseller of their equipment, and must include the letter with their RFQ package. All interested companies shall provide quotations on company letterhead for all items included in the table below.
