- THIS IS A SOURCES SOUGHT NOTICE ONLY. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. The Government will not pay for any information solicited. The Department of Veterans Affairs, Hampton VA Medical Center (VAMC), Hampton Virginia provides diagnostic and therapeutic services in medicine, surgery, rehabilitation medicine, psychiatry, and skilled nursing home care. This facility also provides ambulatory services to those veterans not requiring hospitalization. The Hampton VAMC is seeking contractors interested and capable of providing Non-Emergency Basic Life Support (BLS) and/or Advanced Life Support (ALS) Ambulance Services for the beneficiaries of the Department of Veterans Affairs. Applicable NAICS Code is 621910 with a size standard of $16.5 million and Classification Code V225. Response to this sources sought notice should include the following: 1. Company name, address, and DUNS number 2. POC information 3. business size and socio-economic classification (for example LB, SB, SDVOSB/VOSB etc.) 4. a capability statement/brief description of qualifications and any other pertinent information, which demonstrates ability to meet the above requirement.
