- Statement of Work (SOW) Poison Control Recommendations 1.0 General 1.1 Objective To establish a Base +4 Year Contract for the poison control services such as, but not limited to: the assessment and emergency treatment recommendations on accidental exposures to medications and household substances, plant ingestion, snake, spider and other insect bites and stings, chemical spills, occupational exposures, and drug overdoses. 1.2 Scope of Work Vendor must provide the following: Provide assessment guidance on poison accidents in the following areas: Treatment for accidental poisonings. Information on poison prevention. Assistance with a drug overdose. Bites and stings. Adverse reactions to drugs and chemicals. Toxicity caused by drug interactions. Poisonings from drugs of abuse (cocaine, LSD, etc.) Spider and snake bites. Poisoning of animals. Hazardous spill exposures. Occupational exposure to chemicals. Chronic lead poisoning. Toxic/non-toxic plant information. Educational programs Unlimited number of calls to the Poison Control Center on a toll-free line staffed by trained, certified registered nurses. Provide 24-hour availability to the Poison Control Center Physician Medical Director. 24-hour availability, 365 days/year emergency poisoning management. Customized faxed transmission to supplement information given by the poison information specialists. Management of hazardous chemical exposures. Operation Respond software for identification of hazardous materials transported by railcar in West Virginia. Attendance at hospital health fairs upon request (subject to staff availability). Hospital in-services and continuing education programs on toxicology topics upon request (subject to staff availability). Provide Mr. YUK and other educational materials free of charge. (reasonable quantities) 1.3 Anticipated Periods of Performance The anticipated periods of performance of this contract is as follows: Base Year: July 1, 2021 thru June 30, 2022 Option Year 1: July 1, 2022 thru June 30, 2023 Option Year 2: July 1, 2023 thru June 30, 2024 Option Year 3: July 1, 2024 thru June 30, 2025 Option Year 4: July 1, 2025 thru June 30, 2026 1.4 Place of Performance Hershel Woody Williams VA Medical Center, 1540 Spring Valley Drive, Huntington, WV 25704. 1.5 Applicable Documents Not Applicable. 2.0 Specific Requirements/Tasks See paragraph 1.3 Scope of Work. 3.0 Delivery or Deliverables Not Applicable. 4.0 Meetings Not Applicable. 5.0 Required Travel and Other Direct Costs (ODCs) Not Applicable. 6.0 Government Furnished Property/Equipment/Information No Government furnished property/equipment will be provided.
