Description
1. The purpose of this amendment is to incorporate the following answers to questions submitted by interested vendors: In response to Answer #2 on page 4 of Amendment 0003 :  27101 - Guard ($13.52) as labor category is not in line with the Competency/Training/License/Qualifications (page 7 #4) in which the following is requested, ""A fully trained, experienced screener HEALTH AIDS who possesses a valid TN License...""  Would the government consider 12223 Nurse Assistant III ($13.99) or 12100 Medical Assistant ($14.08), given this is COVID / medically related.  Yes. Labor category used will be changed to 12100 - Medical Assistant and we will not be using Guard I. I have noted Amendment 001 and Amendment 0003, but cannot locate Amendment 0002 on BetaSam. Please advise.  Amendment number 2 was canceled due to error in posting in www.sam.gov. Are the Guard I Screeners required to have security guard training? If so, what level of training? No. Please clarify, Page 9 Hours of Operations states that the Contractor shall be operational on or before the hours of 5 a.m. to 9 p.m., Monday thought Friday, including Federal holidays. The file; copy of sow Screener Number location times at the Mountain Home VA Medical Center list 1 screening Personnel Monday thought Friday 6:00 a.m. 9:00 p.m. (15 Hours). Please clarify the difference between the following. Is the contractor expected use a 16/15 hour day, or should the screening personnel be 2 instead of 1, employees? Please clarify the the number of employees, employee times, and when and how overtime might be employed. The Screener Numbers and locations is for your reference and the vendor must decide the screeners schedule for 1st and 2nd shift. Please use the Bid sheet to submit your quote. Will the Government reimburse the Contractor for the cost of securing PDD skin test and the Annual Flu Shots? VA will provide the contractor employees with skin test, and flu shots. Will the Government accept drivers auto policy as coverage, which will also save the Government money since if the Contractor has to secure a separate policy the cost will only be passed on to the Government? No. Vendor must provide a proof of Certificate of Liability Insurance. 2. The purpose of this amendment is to extend the closing date of the solicitation to 06/04/2021 10:00AM CST.
