Request for Proposal (RFP): 70FA5021R00000006 Date of issuance: 05/28/2021 Due date/time of proposal: 06/11/2021 at 10:00 AM EST End User: DHS, Federal Emergency Management Agency, MWEOC Contracting Agency POC: Chris Barrow Title & Location: Contract Specialist, Mount Weather, VA Telephone No: 540-542-2304 E-mail: christopher.barrow@fema.dhs.gov The Department of Homeland Security (DHS), Federal Emergency Management Agency (FEMA), Mt. Weather Emergency Operations Center (MWEOC) intends to issue multiple Blanket Purchase Agreements (BPA) for repetitive purchases of food products and cleaning supplies to be delivered to the MWEOC facility on an as-needed basis. These products shall meet the specifics listed below. It is anticipated that multiple BPAs will be established based on the various needs. The period of performance is expected to be an inclusive (5) five-year agreement unless otherwise agreed to no longer extend beginning on the date of award in August 2021 through July 2026. BPAs will be established in accordance with FAR 13.303�Blanket Purchase Agreements. The following BPA terms and conditions are mandatory: A. The period of performance for this BPA shall not exceed five (5) years. Each individual purchase under the BPA shall not exceed $995,000.00. The Government is obligated only to the extent of authorized purchases made under the BPA. B. The Contracting Officer will provide a list of individuals authorized to make purchases under the BPA to the Contractor upon award. The Contracting Officer may periodically update this list. C. All deliveries made under the BPA shall be accompanied by a delivery ticket or sales slip that contains the following information: Name of supplier. BPA number. Date of purchase. Purchase number. Itemized list of supplies or services furnished. Quantity, unit price, and extension of each item, less applicable discounts (unit prices and extensions need not be shown when incompatible with the use of automated systems, provided that the invoice is itemized to show this information). Date of delivery or shipment. D. Invoicing: An itemized invoice shall be submitted at least monthly or upon expiration of this BPA, whichever occurs first, for all deliveries made during a billing period and for which payment has not been received. These invoices need not be supported by copies of delivery tickets. All companies doing business with the Government must register their DUNS number in the System for Award Management (SAM) at https://www.sam.gov �Please provide your company�s nine digit DUNS number with any response and ensure the number is currently active in SAM. The basis for award is a Blanket Purchase Agreement with a contractor who can provide the needed food services. To be considered for this agreement, the contractor must demonstrate the�following: Capability of delivering the needed food products and on time Verify their DUNS number is current and active in SAM Provide current pricing for one or more of the food products listed above as well as any associated �costs for delivery (i.e. delivery charges, etc.) Provide the method and process for which pricing food(s) would occur when the Government would place an order. The Government also reserves the right to verify pricing. The pricing of these food products and cleaning supplies shall be, to the best knowledge and belief, as low as, or lower than, the price charged to the Contractor�s most favored customers for comparable quantities under similar terms and conditions, in addition to any prompt payments. An example of this method would be providing the Government and OPIS report (https://www.opisnet.com/). In addition to the items above that the Contractor must demonstrate, the Government will consider Past Performance information, which may be based on information obtained from the offeror�s supplied references through telephone interviews, the PPIRS database, or information obtained by the Government from other sources.
