- 36C24921Q0351 INTENT TO SOLE SOURCE The Department of Veterans Affairs, Network Contracting Office 9, intends to award a sole-source, contract to Midwest Security Services, INC, 4050 Benfield DR, Dayton, OH, 45429 for the Louisville Robley Rex VAMC, 800 Zorn Avenue, Louisville, KY 40206. FAR 6.302-1 Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements, applies to this procurement."" The contract is expected to be awarded under FAR 13.106-1 (b). Period of Performance: Base Year: 06/30/2021-10/31/2021 NAICS code: 561621 SBA Size Standard: $22M PSC: DJ10 STATEMENT OF WORK SECURITY CAMERA INSTALLATION INPATIENT PHARMACY SCOPE: The inpatient pharmacy requires the installation of three (3) cameras to cover pharmacy work areas. One (1) camera placed to cover the pharmacy entrance doorway into the pharmacy work area. This camera shall be adjustable to appropriate focus plane and cover the doorway and entrance area around it. One (1) camera placed to cover the pill sorting and packing machine. This camera shall provide 360-degree coverage of the area and will allow for direct view of employee actions in the area. One (1) camera placed over the inpatient pharmacy vault/Pyxis machine. This camera Shall provide 360-degrees coverage of the area and will allow for direct view of employee actions in the area to include keystrokes on the computer keyboard. REQUIREMENTS: All cameras shall be licensed to operate on the Milestone Software program used by Police Services. Each camera will be licensed on that system, with license to be provided with install. Each camera Shall be cabled into the police camera network and terminated at the appropriate switch head. Each camera will be adjusted to provide the required coverage and correct depth of field and focus for the area of coverage. Each camera will be registered into the Milestone software package and loaded onto the monitoring system. The contractor shall provide all services, supplies and related equipment to complete the installation. INSPECTION AND ACCEPTANCE: Louisville VAMC Police Service personnel will review all services and deliverables to determine acceptability. If neither the services nor deliverables conform to the contract requirements or Police Service standards, the Police Service may require the Contractor to repeat these services at no increase in the contract amount. When defects in services or deliverables cannot be corrected by performance, the Police Service may require the Contractor to take necessary action to ensure that future performance conforms to contract requirements. If the Contractor fails to promptly perform the services again or take the necessary action to ensure future performance in conformity to contract requirements, the Police Service may (1) by contract or otherwise, perform the services and charge the Contractor any cost incurred, or (2) terminate the contract for default. POLICE SERVICE RESPONSIBILITY: Police Service will provide the necessary support for contracting personnel to access areas of service, connection closets and the camera monitoring station.
