THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6-STREAMLINED PROCEDURES FOR EVALUATION AND SOLICITATION FOR COMMERCIAL ITEMS-AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE, THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. OFFERS ARE BEING REQUESTED, AND A WRITTEN SOLICITATION DOCUMENT WILL NOT BE ISSUED. This is solicitation, no. W81K0221Q0110; Purchase Request; 0011629528, issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-02.� The associated North American Industrial Classification System (NAICS) code for this procurement is 611710; Size Standard for small business is $16.5M. �RFQ is being issued as Small Business Set-A-Side.� �This requirement is a Base with 4 Option Years for Training Management and Evaluations Resident / Fellows tracking with the following Line Items listed below: � CLIN 0001������ Base Yr � Web-based Class 3 Institutional Fee 300 or less residents/ fellows, Medical Training Management.�� Period of Performance: 7/1/21 through 9/30/2021������������������������ QTY 01 CLIN 0002������ Option Yr (1) � Web-based Class 3 Institutional Fee 300 or less residents/fellows, Medical Training Management.� Period of Performance: 10/1/2021 through 9/30/2022������� QTY 01 CLIN 0003������ Option Yr (2) � Web-based Class 3 Institutional Fee 300 or less residents/fellows, Medical Training Management.� Period of Performance: 10/1/2022 through 9/30/2023������� QTY 01 CLIN 0004������ Option Yr (3) � Web-based Class 3 Institutional Fee 300 or less residents/fellows, Medical Training Management.� Period of Performance: 10/1/2023 through 9/30/2024������� QTY 01 CLIN 0005������ Option Yr (4) � Web-based Class 3 Institutional Fee 300 or less residents/fellows, Medical Training Management. Period of Performance: 10/1/2024 through 9/30/2025�������� QTY 01����������� ����������� STATEMENT OF WORK � Residency Evaluation Management Scope of Work � 1.1 Contractor shall provide an On-line Service Tool which allows Medical Interns and Residents to unify required data into a centralized Data Warehouse to complete tasks historically performed using multiple/incompatible methods. � 1.2 Contractor shall provide Software, Licenses, and Use for up to 300 Residents � 1.3 Program and Data Base must be compliant under the Accreditation of Council of Graduate Medical Education (ACGME). � 1.4 Contractor shall provide data management through one common interface. � ����������� 1.5 The Data Management Tool shall provide the following as a minimum the following tasks: ����������� 1.6 Scheduling ����������� 1.7 Procedure Logging ����������� 1.8 Evaluations input ����������� 1.9 Monitoring Conference Attendance ����������� 1.10 Duty Hours ����������� 1.11 General Personnel Tracking ����������� 1.12 Curricular milestones ����������� 1.13 The contractor shall provide an off-site data base which allows the Graduate Medical Education ����������� (GME) Faculty at Madigan Army Medical Center, Tacoma WA to use a unified database. ����� ����� 1.14 The contractor shall also be required to respond to customer needs in a timely, responsive, and flexible manner. Key objectives: � Vendor / Software shall; � 2.1 Automatically generate evaluations for all people assigned to a rotation (360 degree evaluations) � ����������� 2.2 Allow evaluators to submit evaluations anonymously ����������� 2.3 Print individual evaluations and revise as necessary ����������� 2.4 Require electronic signature of both evaluators and/or subjects ����������� 2.5 Generate compiled reports for any time period with: Peer or departmental average, individual average minimum and maximum scores, standard deviation, listing of ALL scores, ability to include comments, ����� automatic email notifications to users and supervisors, ability to generate reports showing evaluations that were not completed, allow people to choose to evaluate individuals not assigned to them, report department configured duty hour rule validations, report violations designed to detect potential and past violations hourly report, hourly report by rotation, duty hours entered may be used to facilitate Integrated Risk. ����������� Information System,� (IRIS) calculations, ability to assign curriculum for a particular rotation, ability to assign curriculum for a particular status type, residents will receive email reminders to view their curriculum prior to the start of the rotation, residents may sign off electronically indicating they�ve reviewed the curriculum Specific Tasks � 3.1 Contractor shall be responsible for Setup, Training and Software Modifications � ����������� 3.2 Contractor shall provide training via teleconference or phone to Program Administrators for use of the Software. ����������� 3.3 Contractor shall set up an account on their servers for this facility which will include the following: ability to log procedures via the Internet or Palm Pilot device,� customize procedure names, locations and roles, produce standard reports, create your own custom evaluation forms, evaluate students, residents, ������� faculty and the rotation, generate electronic reminders, log conference attendance via the Internet or Palm Pilot device, customize conference names and core content, specify attendance compliance, maintain all department call and clinic schedules, produce and configure custom schedule printouts, produce department annual schedules and department monthly schedules (student, resident and faculty) by assignment, physician, program year (PGY) level, clinic. ����������� ����������� 3.4 Download Personal Schedules to PDA/Laptops. ����������� 3.5 Import Electronic Residency Application Service (ERAS) ����������� 3.6 Exported files. ����������� 3.7 Tracks a physician from undergraduate school through alumni ����������� 3.8 Creates IRIS importable files. ����������� 3.9 Contractor shall be responsible for providing troubleshooting/debugging assistance as necessary. 4. � System Configuration and Accessibility ����������� 4.1 System must be available 7 days a week, 24 hours a day, 365 days a year, unless�� otherwise agreed to ����� by both parties or scheduled maintenance. ����������� 4.2 Agreements must be reached 72 hours prior to system down time. ����������� 4.3 Vendor must approve the use of Internet Explorer 5.5 and all subsequent releases of Microsoft Internet Explorer Browser. ����������� 4.4 All upgrades to the software, licensed by Licensee, will be provided to Licensee free of charge, providing they are current with all applicable maintenance fees. ����������� 4.5 Vendor must provide 128 bit encryption utilizing Verisign or other verification method as required by ����� HIPAA (See Privacy and Security Health Information Clause attached). 5.� Training Requirements ����������� 5.1 All Training must include but not limited to the following: Log on procedures, data entry, program customization, report creation, electronic record application system and record management. ����������� 5.2 This Training will meet the requirements to be certified as Accreditation Council for Graduate Medical Education (ACGME) ����������� 5.3 Contractor will provide the following Training Aids:� On-line access to their resident management system (Passwords, software, and Items needed for Student Login) ����������� 5.4 Ability to allow evaluators to submit evaluations anonymously ����������� 5.5 Print individual evaluations and revise as necessary ����������� 5.6 Require electronic signature of both evaluators and/or subjects ����������� 5.7 Generate compiled reports for any time period with peer or departmental average, individual average and minimum and maximum 6.5 Sound System ����������� 6.6 Network Link to Internet 7.�� Other Contractor Services ����������� 7.1� Restoration of User Data: If corruption or deletion of user data has occurred due to hardware, operating system, software malfunction, or operator error, during the term the vendor will assist Licensee in ���� restoration of such data upon the� following conditions: Contractor shall be required to keep adequate backups of the software and all Licensee data stored on their server(s), contractor shall neither be responsible nor liable for the loss or corruption of user data regardless of the reason for such loss. EVALUATION OF FACTORS: � Award will be made based on Technically Acceptable, then Lowest reasonable price.� Offeror submission must include enough information to clearly demonstrate to the Contracting Officer whether or not product meets the Performance Work Statement (PWS).� The evaluated price will be the total price of the quote, to include all fees as indicated, cold storage, freight charges, & technician escort travel as required in the solicitation.
