Description

The Department of Veterans Affairs (VA) San Francisco Community Based CBOCs require temporary Locum Primary Care Physician services at the San Francisco VA Health Care System�s (SFVAHCS) Community Based Outpatient Clinics (CBOCs). The Locum Physician(s) shall provide ongoing primary care services for chronic medical problems commonly seen in the veteran population; included are such disorders as diabetes, hypertension, chronic obstructive pulmonary disease, degenerative arthritis, depression, congestive heart failure, angina and pain. Judicious use of laboratory and x-ray services as well as appropriate referral and consultation is obtained in accordance with the terms and conditions contained herein.� Period of Performance: Base year plus 4 option years from 1/1/2021 to 10/31/2026 Base period: November 1, 2021 through October 31, 2022 Option Year 1: November 1, 2022 through October 31, 2023 Option Year 2: November 1, 2023 through October 31, 2024 Option Year 3: November 1, 2024 through October 31, 2025 Option Year 4: November 1, 2025 through October 31, 2026
