- THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. VHA RPO West NCO 17 Health Care Resources Branch I is planning to solicit for a vendor who can provide On-Site Inpatient Hemodialysis Services for Veterans within the area of the Thomas E Creek VA Medical Center. The Thomas E Creek VA Medical Center is looking for a Contactor that can provide all supervision, personnel, supplies, transportation, and equipment to perform inpatient acute hemodialysis treatments. VHA RPO West NCO 17 Health Care Resources Branch I is conducting a market survey to determine the existence of potential small business concerns (e.g., HUBZone, 8(a), SDVOSB, Woman Owned, etc.) with the capability to fulfill this acquisition. Place of Performance: Department of Veterans Affairs Thomas E Creek VA Medical Center 6010 Amarillo Blvd West Amarillo, TX 79106-1991 Type of Contract: If this sources sought notice leads to a future solicitation, the Government intends to award a firm-fixed price contract. The performance period shall be one (1) base year with four (4); one (1) option years. Interested parties shall provide a brief overview of their firm's capabilities and past experience with projects similar in scope to this project. All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments. Each capability statement submittal shall also include the following business information: a. Organization Name b. Organization Address c. Organization DUNS Number d. Point of Contact (including name, title, addresses, telephone number, fax number and Email address) e. Business size under NAICS 621492 f. Socio-economic status (IF SDVOSB or VOSB - provide SDVOSB or VOSB CVE verification) g. Types of dialysis services and equipment the organization can provide Firms shall identify whether or not they are a small business or other than small business in accordance with the size standard for NAICS code 6212492. If another NAICS code may be applicable, please identify the NAICS in your capability statement. All information must be received no later than June 28, 2021 by 2:00 PM ET.
