- Department of Veterans Affairs Program Contracting Activity Central (PCAC) Notice of Intent to Sole Source: High Alert Medication Safety Self-Assessment (Proactive Risk Assessment). Background: The Department of Veterans Affairs and the Office of Nursing Services would like to contract customized survey services to monitor the improvement in the use of high alert medication. This assessment will serve as a proactive risk assessment of medication use. The VA has a need to monitor the use of high alert medication. The Institute for Safe Medication Practices (ISMP) is a Patient Safety Organization (PSO) as designated under the patient safety improvement act of 2005. ISMP has a national level / FDA funded website application that has self-assessment questions already built into the application (see attachment). About 100 sites did the survey in 2018 and the use of this tool again will assist in the tracking and trending of the progress toward safety improvement, but it will also include ALL sites Scope of Work: The Contractor shall provide survey administration and data compilation services. They will grant VA sites access to the website application and accept data into the application. The sites will have to use their facility number to identify what location is conducting the survey and to allow collation of data by VISN also. The contractor will assist the sites with any technical issues and extract the data for VA for national and VISN use. Submittal Information: As stated above, this is an Intent to Sole Source. However, any responsible source who believes it is capable of meeting the requirement may submit a capability statement, proposal, or quotation, which shall be considered by the agency, only if received by the closing date and time of this notice. A determination not to compete the proposed requirement based upon the responses to this notice is solely within the discretion of the government. Responses to this notice are due on or before June 4, 2021 at 11:59AM EST, to Adeitra Jimmison via email at Adeitra.Jimmison@va.gov. All responses must include the following information: Company name, Cage code, Dun & Bradstreet number, Company address, Point of contact name, Phone number, Fax number, GSA contract (if applicable) and email. The subject line for correspondence should clearly display: High Alert Medication Safety Self-Assessment (Proactive Risk Assessment).
