- Description
- The Albuquerque Area Indian Health Service, Santa Fe Service Unit has a requirement for Medical Gases and Cylinder Rental� Services. This is a combined synopsis for commercial items or services prepared in accordance with format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is 75H70721Q00024 and is issued as a Request for Quote (RFQ), unless�otherwise indicated herein. The solicitation document and incorporated provision and clauses are those in effect through Federal Acquisition Circular 2021-04. The associated North American Industrial Classification System (NAICS) code for this procurement is 325120 with a business size standard 1000. This requirement is a Set-Aside for Small Business Indian Firms and only quotes from qualified offerors will be considered. Offers will be solicited from Small Business Indian Firm concerns. If we do not receive enough competition and/or reasonably priced offers from the Small Business Indian Firm set-aside, we will then evaluate quotes and consider for award to other Small Business concerns. Any acquisition resulting from this cascading set-aside will be�from such a concern as follows: First Tier: Small Business; Second Tier: Service Disabled Veteran-Owned Business; Third Tier: HUBZONE and Fourth: All Other Large Business Concerns. ONLY ONE AWARD WILL RESULT FROM THIS SOLICITATION. FOB Destination shall be for the following�Albuquerque Area Indian Health Service facility: SANTA FE SERVICE UNIT Attn: Housekeeping Department 1700 Cerrillos Road Santa Fe, New Mexico 87505 **SEE ATTACHED PRICE SCHEDULE AND SCOPE OF WORK FOR ADDITIONAL DETAILS** This solicitation will result in a Single Award with One (1) Base Year from date of award and Four (4) 12-month option periods based on Satisfactory Performance ratings. State Taxes for labor/service costs (The Indian Health Service is exempt from New Mexico State Taxes for materials and supplies.) Quote must be free on board (FOB) Destination CONUS (Continental U.S.), which means that the seller must deliver the goes on its conveyance at the destination specified by the buyer, and the seller is responsible for the cost of shipping and risk of loss prior to actual delivery at the specified destination. This solicitation requires registration with the System of Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov All responsible offerors that respond to this solicitation must submit their quotes by June 16, 2021, by COB 3:30 p.m., (MDT).
