Description
This RFI (Request for Information) is issued solely for information and planning purposes and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. This notice is published to conduct market research to determine if there is a sufficient number of certified Service Disabled Veteran Owned Small Businesses (SDVOSB), Veteran Owned Small Businesses (VOSB), Small Businesses (SB), or Large Business capable of performing the requirements to warrant a socio-economic set-aside or full & open competition. It is strongly recommended that interested parties register in the US Department of Veterans Affairs Vet Biz database, the Small Business Administration (SBA) database and the System for Award Management (SAM), as applicable. Registration may be accomplished at http://www.vip.vetbiz.gov/, www.sba.gov/ and https://beta.sam.gov respectively. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) company name and address, point of contact with phone number, and DUNS number; (2) documentation relating to capability of performance (3) intentions of subcontracting program to include set-asides if any; (4) proof of registration in the VA Vet Biz or SBA websites or intention of registration prior to submission of offers to a solicitation; (5) If you are a distributor a statement from the manufacturer that your company is an authorized distributor; (6) a statement to inform the government if you are a manufacturer or distributor; (7) any information regarding FSS Schedule availability; (8) SAM Registration (9) any other pertinent company documentation. The response date to this Sources Sought notice is June 17, 2021 at 3:00pm MST. This market research is for informational and planning purposes only to determine if a socio-economic set-aside, or full & open competition, or a sole-source procurement is appropriate. The Government will not pay any costs for responses submitted. Electronic submissions are acceptable via nakia.vance2@va.gov, and are highly preferred. NO ADDITIONAL INFORMATION IS AVAILABLE OTHER THAN WHAT IS NOTED HEREIN. ALL FIRMS ARE INSTRUCTED NOT TO CALL THE INDIVIDUAL VA MEDICAL CENTERS REQUESTING MORE INFORMATION Statement of Work Driver Rehabilitation Training for Phoenix VA Health Care System (PVAHCS) and Southern Arizona VA Healthcare System (SAVAHCS) 1.Estimated Annual Quantities of Training and Evaluations Driver Evaluations approximately 250 per year for Phoenix Training approximately 150 veterans per year for Phoenix Driver Evaluations approximately for 50 per year for Tucson Training approximately 30 veterans per year for Tucson These quantities are estimates only. The Government reserves the right in increase or decrease quantities based on the actual yearly need. 2.Contract Period: This contract shall not to exceed five (5) years. The Contract will be for one (1) Base year with four (4) Option years. 3.Locations: Phoenix VA Health Care System (PVAHCS) 650 E Indian School Road Phoenix, AZ 85012 Southern Arizona VA Health Care System (SAVAHCS) 3601 S. 6th Ave Tucson, AZ 85723 4.Description of Services Definition: Driver rehabilitation for the disabled, within the Department of Veterans Affairs is designed to provide professional evaluation and instruction for eligible veterans in the safe, competent utilization of special add on equipment, and mastery of specific skills and techniques to effectively drive a motor vehicle, independently, and in accordance with Arizona State Department of Motor Vehicle (DMV) regulations. The following objectives are to be accomplished: a. The patient will acquire specific knowledge in all areas of the driver training course including didactic theory and practical experience. b. Psychomotor improvements in handling segments of the driver training task will be noted by comparison of function before and after completion of the training course c. The patient will maximize independence in all aspects of driving d. Supporting adaptive equipment will enable patients to mainstream themselves into society. e. The C&A requirements do not apply, and that a Security Accreditation Package is not required. 5. Staff Qualifications The Contractor shall have at least one contract driver rehabilitation specialist in the organization that is licensed, registered, or certified by a professional organization for this type of service. All specialists must have a minimum of a Baccalaureate Degree from a Regionally Accredited University in adapted physical education, kinesiotherapy, physical therapy, occupational therapy or a specifically related rehabilitation therapy area or certified Driver Rehab specialist. Completion of the National Driver Rehabilitation Specialist Examination is preferred. 6. Scheduling & Documentation a. The PVAHCS staff will contact contractor when a consult for driver rehabilitation training is ordered. b. The Veteran/patient shall be scheduled for training by the contractor within fourteen days of referral. c. All evaluations shall be completed within 30 days after the evaluation unless veteran prefers a later time, then contractor will need to refer back to VA for requested permission for service extension.
