- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice.� This announcement constitutes the only solicitation; quotes are being requested in response to this notice and a written solicitation will NOT be issued. This solicitation is being issued as a Request for Quote (RFQ) # 246-21-0068.��Submit only written quotes for. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05.� The associated NAICS code is 811219. This is a 100% small business set-aside. This RFQ contains Two (2) Line Item: CLIN DESCRIPTION QTY UNIT Unit Price TOTAL 0001 09/21/2021 � 09/20/2022 � EchoCardiogram Interpretation Services according to the attached SOW. 650 Each � � 0002 09/21/2021 � 09/20/2022 � Stress EchoCardiogram Interpretatations in Radiology Department according to the attached SOW. 10 Each � � � PERIOD OF PERFORMANCE: ��09/21/2021 � 09/20/2022 Vendor Requirements: ��SEE ATTACHED Statement of Work (SOW) Submit Quotes no later than: 06/11/2021 @ 4:30PM CST� to the Following Point of Contact: Misti Bussell, Purchasing Agent, via Email: misti.bussell@ihs.gov. � Questions and Answers: Due by 06/10/2021 by 9 PM CST and will be posted by 06/10/2021 @ 3 PM CST via Email to: misti.bussell@ihs.gov Evaluation:� FAR 52.212-2 Evaluation -- Commercial Items (completed as follows): (a) The Government will award a firm fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Lowest Price Technically Acceptable - The contractor shall provide standard transthoracic echocardiogram interpretations that include Left and Right Ventricle, Left and Right Atrium, Aortic Mitral, Tricuspid and Pulmonic Valves, Aorta and Pericardium findings. - Contractor shall provide a 48 hour turn around time on routine reports and 24 hour turn around time on Urgent reports beginning upon completion of exam. - Vendor will be evaluated on past-history performance. VENDORS SUBMITTING OR EQUAL ITEMS MUST SUMBMIT DESCRIPTITVE LITERATURE SHOWING HOW THEIR PRODUCT MEETS OR EXCEEDS THE REQUIREMENTS BEING SOLICITED � INCLUDES SERVICE CONTRACT.
