THIS IS A SOURCES SOUGHT ANNOUNCEMENT TO BE USED FOR MARKET ANALYSIS ONLY. NO AWARDS WILL BE MADE FROM THE RESPONSES TO THIS SYNOPSIS. NO SOLICITATION WILL BE AVAILABLE AT THIS TIME. THE PURPOSE OF THIS ANNOUNCEMENT IS TO GAIN KNOWLEDGE OF POTENTIAL QUALIFIED CONCERNS DOING WORK UNDER NAICS 621999. ANY POTENTIAL COMPETITIVE SOLICITATION(S) THAT MAY PROCEED FROM THIS NOTICE WILL BE POSTED TO FEDBIZOPPS. ALL INTERESTED OFFERORS WILL HAVE THE OPPORTUNITY TO RESPOND TO A SOLICITATION ANNOUNCEMENT AT A LATER TIME IF AND WHEN A SOLICITATION IS ISSUED. The proposed service is for the FedStrive Advantage Services�� � The following Statement of Work lists those services and associated costs that will be provided to your agency, per your request. FOH will provide the highest quality service and encourages your feedback to the FOH Program Point of Contact, as listed in this document. Scope of Services Details: Clinical Health Services: Fed Strive Advantage Service Plan Background and Purpose The Federal Occupational Health (FOH) FedStrive Advantage (FSA) services are designed to support and maintain a healthy and maximally productive workforce for federal customer agencies. The services are provided onsite by licensed, BLS certified, registered nurses (RNs) and include emergency medical response for life-threatening conditions which may require basic life support, the use of an automated external defibrillator (AED), or administration of emergency medications while awaiting the arrival of local EMS. In addition to emergency care, FSA provides convenient onsite care for minor illness and injury to minimize unnecessary absences that may otherwise result as well as basic preventive health services and education to allow workers to maintain or improve their health in order to prevent the development or progression of chronic disease. Personalized health counseling is available based on individual health screening information collected. FSA services are generally provided in health clinics located within federal agency buildings, but may include other locations on an agency campus, in the setting of emergency response, and/or educational programs. The clinic may service a single agency or multiple agencies. Clinic hours vary by location, customer preference, and the size of the population served by the clinic. While a dedicated RN is assigned to an FSA clinic location, the site may be staffed intermittently by back up nurses, as appropriate.. Emergency response services provided as part of the FSA plan include response to life-threatening emergencies by the clinic nurse equipped with emergency supplies including an AED during clinic hours of operation. FOH offers a separate public access (lay responder) AED program to federal agencies wishing to establish a program with AEDs located outside of the clinic. For agencies that participate in both the FSA service plan and the FOH AED program service plan it should be noted that the duties of the nurse provided under FSA do not include serving as the agency POC for the AED program including device management, lay responder and training coordination or responding to AED program requests. Available Services: FSA services are provided at a per capita rate for individual customer agencies that may vary based on the size of the agency population, the clinic hours of operation, and according to whether the clinic serves a single or multiple agencies. The available services include: �Emergency Response including CPR, first aid or use of the AED; �Walk-in care for minor illnesses and injury that may be work- or non-work related; �Routine adult immunizations according to guidelines from the Centers for Disease Control (CDC): including Tetanus-diphtheria (Td), Tetanus-diphtheria-acellular pertussis (Tdap), Influenza, and pneumococcal (pneumonia) vaccines; �Destination specific travel information from the CDC to include RN consultation for questions about the information provided; �Care prescribed by an employee�s personal physician including administration of medications, administration of allergy immunotherapy, blood pressure and glucose (blood sugar) monitoring, wound care, and periodic bedrest. (Orders for care prescribed by outside physicians are subject to FOH physician review and approval); �Fed Strive Online Health Portal including a comprehensive Health Risk Appraisal (HRA), individualized tracking tools and disease specific risk assessments; �Health education programs including at least three 30 minute programs provided annually onsite at the agency location; �Health screenings including vision, blood pressure, diabetes, cholesterol, and other risk factors for heart disease; �Individualized health coaching and counseling including targeted interventions based on individual risk factors; �Specialized screenings such as bone density or cancer screenings using mobile equipment where available and supported by customer agency �Management of designated lactation space located within the clinic space to include regular cleaning of the room and any agency provided equipment. �Clinic staff participation in occupant emergency planning with other tenant agencies; �Annual program review to assist management in identifying basic safety, health, and wellness opportunities. Additional Services Requests for services away from the clinic are available on a fee for service outreach basis. Reporting FOH provides a monthly service and charge report to the customer agency POC listed on the interagency agreement/statement of work. FOH may also provide service specific utilization reports and an executive summary of the online health appraisal in agencies with more than 25 participants. FOH may assist customer agencies in applying the information collected to targeted educational programs. Eligibility FOH�s legal authority only authorizes the provision of occupational health and wellness services to federal employees as defined in this SOW at predetermined locations. Since the general public and contractor staff may also be present in federal buildings, FOH will provide emergency care to all individuals as described in this SOW in the event of a life-threatening emergency; however, contractors and the general public may not otherwise receive routine convenience care services within FOH clinics unless a specific exception is granted in advance. This limitation also applies to FOH clinics located in federal buildings that may house or contain special populations including, but not limited to, federal detainees and minors attending childcare programs. � 2. Responses to this sources sought will be used by the Government to make an appropriate procurement decision. All firms (large or small business) should respond to the project if they are interested in performing it. 3. Please note that this notice is not a commitment on the part of the Government to award a contract as a result of this notice or to pay for any information received. This synopsis neither constitutes a Request for Proposal or Invitation for Bid, nor does it restrict the Government to an ultimate acquisition approach. The Government should not construe this Sources Sought Synopsis as a commitment for any purpose. The Government does not intend to rank submittals or provide any reply to interested firms. 5.
