1. This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. 1.A. Project Title: VA Annual Boiler Maintenance & Service. 1.B. Description: VA San Diego Healthcare System has a requirement to provide annual maintenance services, and emergency repair service for all boilers, deaerator tank and boiler plant controls at the VASDHS. 1.C. Project Location: VA San Diego Healthcare System, 3350 La Jolla Village Dr. San Diego, CA 92161 1.D. Solicitation Type - Number: Request for Quotes (RFQ) - 36C26221Q0865 1.E. Set Aside: This solicitation is 100% set aside for Service-Disabled Veteran Owned Small Businesses IAW Public Law 109-461, 38 USC Section 8127 1.F. Applicable NAICS code: 238220, Plumbing, Heating and Air Conditioning Contractors. 1.G. Small Business Size Standard: $16.5 Million 1.H. Type of Contract: Firm Fixed Price 1.I. Period of Performance: The period of performance shall be for 1 year. 1.J. Wage Determination: SCA Wage Determination No. WD 2015-5635, rev 14 1K. Key solicitation milestones are: 1.K.1. A site survey will be conducted at 10am Thursday, June 17, 2021. Participants will meet in building 2 at the VASDHS. All participants must have face covering and practice social distancing. Please email Jonathan.Ford1@va.gov to indicate that you are participating in the site survey, also an update on the day, time, and location will be provided, should there need to be a change. 1.K.2. Submit any questions regarding this procurement via email to Jonathan.Ford1@va.gov no later than 1000 am Thursday, June 26, 2021. No questions will be answered after this date/time unless determined to be in the best interest of the Government as determined by the Contracting Officer. Telephonic questions will not be accepted or returned. RFI responses will be responded to as necessary in amendment format which will be posted on beta.sam.gov 1.K.3. Submit written offers in accordance with the following instructions, as well as Addendum to FAR 52.212-1, Instruction to Offerors. Oral offers will not be accepted. Complete and submit enclosure 2 via email no later than 3:00pm Thursday, July 1, 2021 to Jonathan.Ford1@va.gov. Ensure the following are completed and/or included in the quote packet: 1.K.3.A. Vendor Information 1.K.3.B. Acknowledgement of amendments 1.K.3.C. Concurrence/non-concurrence with solicitation terms, conditions, and provisions 1.K.3.D. Schedule of services 2. Statement of Work. 2.A. Background: The Department of Veterans Affairs San Diego Healthcare System (VASDHS) has a requirement to provide Boiler Annual Maintenance and Service. Malfunctioning burners or controls can cause catastrophic events resulting in injuries and death and massive property damage. Inaccurate or inadequate data management and monitoring systems can fail to warn of unsafe or inefficient performance. The purpose of this contract is to ensure Boiler plan safety, reliability, efficiency, and equipment longevity at the VASDHS. This can only be achieved by regular inspections, testing and calibration by expert technicians. 2.B. Scope. Contractor shall furnish all labor, materials, supplies, tools, equipment/tools, transportation, personal protective equipment, supervision, and qualified personnel to for the annual maintenance of all boilers, feedwater deaerator tank and boiler plant controls as described in this scope of work. For the Boiler Annual Maintenance, Contractor Shall: 2.B.1. Inspect all actuators & motors for abnormal operation 2.B.2. Inspect all lights, indicators, & alarms for functionality 2.B.3. Inspect & clean all burner traps & strainers 2.B.4. Inspect atomizing media equipment 2.B.5. Inspect boiler & burner components for wear 2.B.6. Inspect burner flame pattern 2.B.7. Inspect firing rate control 2.B.8. Inspect flue, vent, stack, & outlet dampers 2.B.9. Inspect fuel nozzles & fuel outlet orifices 2.B.10. Inspect fuel train(s), regulator(s), & valves 2.B.11. Inspect diffuser positioning 2.B.12. Perform a pilot turndown test 2.B.13. Perform leak test on pilot & fuel train(s) 2.B.14. Test & reset combustion 2.B.15. Test & verify firing rate control set points 2.B.16. Test flame failure safety shutdown timing 2.B.17. Test fuel train(s) interlocks 2.B.18. Test high & low fuel temperature/pressure interlocks 2.B.19. Inspect gauges, monitors, & indicators 2.B.20. Inspect igniter for damage & wear 2.B.21. Inspect inlet & outlet dampers 2.B.22. Inspect instruments & equipment settings 2.B.23. Inspect linkage, drive arms, & damper connections for wear 2.B.24. Inspect main fuel safety shutoff & vent valves for leakage 2.B.25. Inspect pilot & main fuel flame signal strength 2.B.26. Inspect pilot line, regulators, & valves 2.B.27. Inspect the blower motor operation 2.B.28. Inspect the diffuser & burner components for ear 2.B.29. Test atomizing medium interlocks & set points 2.B.30. Test burner position interlocks 2.B.31. Test combustion air proving switch 2.B.32. Test damper position interlocks 2.B.33. Test flame failure detection system for pilot & main fuel(s) 2.B.34. Test operating & high limit control functionality 2.B.35. Test trial for ignitions & full sequence timing 2.B.36. Inspect boiler for visible signs of hot spots & discoloration 2.B.37. Inspect attaching mechanisms & open all access doors 2.B.38. Inspect fireside of boiler & clean debris or soot as required 2.B.39. Inspect the refractory & insulation for wear 2.B.40. Drain boiler, open manholes & handholes for inspection 2.B.41. Inspect & flush waterside of boiler 2.B.42. Inspect PV for cracks, visible corrosion & scale 2.B.43. Inspect the feed water valve & controls for operation 2.B.44. Open & inspect internals of low water cutoff equipment 2.B.45. Blow down the gauge glass assembly 2.B.46. Inspect & flush pressure control tree as needed 2.B.47. Inspect blowdown valves & equipment for leakage & wear 2.B.48. Inspect the safety relief valves for leakage 2.B.49. Inspect water column & gauge glass for wear & etching 2.B.50. Perform a slow drain test on low water cutoff 2.B.51. Test water column water level ports 2.B.52. Safety relief valves-SRV- must be replaced annually 2.C. Scope (Continued). For the Deaerator Annual Maintenance, Contractor Shall: 2.C.1. Drain and open tank for inspection 2.C.2. Inspect internals (visually) for corrosion 2.C.3. Inspect internals (visually) for possible component failure 2.C.4. Remove plugs from equalizing lines 2.C.5. Open high- and low-level alarm float bowls 2.C.6. Flush tank and control the lines 2.C.7. Inspect safety release valves (SRV) and replace annually. 2.C.8. Inspect and thoroughly clean feed pump suction lines 2.C.9. Inspect pump couplings for proper alignment and/or excessive wear 2.C.10. Check the pump discharge pressure 2.C.11. Provide upgraded gaskets 2.C.12. Reassemble the deaerator tank 2.C.13. Check pump motor amperage draws 2.C.14. Reset pressure and temperature controls to obtain desired set point 2.C.15. Perform oxygen test (Deaerator tanks only) 2.C.16. Record control settings and perform operational tests 2.C.17. Assure that tank is operating properly and safely 2.D. General Requirements 2.D.1. Contractor shall have ready access to all parts and materials. 2.E. Specific Requirements 2.E.1. The Contractor shall use only new Original Equipment Manufacturer (OEM) or OEM approved parts. All parts shall be of current manufacture and have complete versatility with the presently installed equipment. All parts shall perform identically to the original equipment specifications. Parts removed from another system, rebuilt and/or used, shall not be installed without specific approval by the CO and the COR. All defective parts replaced become the property of the Contractor, unless otherwise specified by the COR. 2.E.2. Contractor shall conform to following standards, policies, regulations, governing agency, and any other local, state, or Federal guidelines that are not listed here. i. OSHA Regulations, www.osha.gov ii. NFPA, www.nfpa.org 2.F. Qualifications: Technicians must demonstrate familiarity with and ready access to the most recent version of the following references: 2.F.1. Contractor shall obtain all necessary licenses required to perform this work 2.F.2. The Contractor must have been conducting business for a minimum of 10 years with a minimum of 10 years verifiable experience performing boiler maintenance and repair services. 2.F.3. The Contractor must have and maintain in good standing, a Certificate of Authorization, and an R symbol stamp from the National Board of Boiler and Pressure Vessel Inspectors for the repair and/or alteration of boilers, pressure vessels. 2.F.4. The contractor must be licensed and authorized to work on Clever Brooks boilers. 2.F.5. Technicians must have completed at least a one-year trade school and have five years successful experience (verifiable) in this field. The experience must be largely with institutional and boiler plants similar in design, scope, and magnitude to the VASDHS. The COR may define and define and accept equivalent qualification. 2.F.6. Technicians must demonstrate familiarity with and ready access to the most recent version of the following references: 2.F.6.1. NFPA 85, Boiler and Combustion Systems Hazards Code including applicable appendices. 2.F.6.2. VHA Boiler Plant Safety Device Testing Manual. 2.F.6.3. VHA directive 1810, Boiler and Boiler Plant Operations. 2.F.6.4. Written recommendations of the equipment s manufacturer. 2.F.6. Rules of the Station: Contractor shall comply with all rules of the facility, including security badging, non-smoking and privacy, and reporting to the COR upon arrival at the facility. 2.F.7. Smoking Policy. Smoking is strictly prohibited on the grounds of any VHA facility. Per VHA Directive 1085 dated March 5, 2019, it is VHA policy that all VHA health care facilities, including hospitals, community clinics, administrative offices and Vet Centers, will be smoke-free for patients, visitors, contractors, volunteers, and vendors effective October 1, 2019. There will no longer be designated smoking areas. 2.F.8. Contractor shall obtain a Contractor I.D. Badge from the VA police. All Contractor personnel are required to wear I.D Badge during the entire time on VA campus. 2.F.9. Possession of weapons is prohibited. Enclosed containers, including tool kits, are subject to search. Violations of VA regulations may result in citations answerable in the United States (Federal) District Court. 2.G. Performance Monitoring. The Contracting Office Representative (COR) will monitor contractor performance. The COR will certify the work was done in accordance with the SOW. COR will inspect the system once the service has been completed and the system is returned to normal operation. The Contractor shall comply with all applicable regulations and guidelines of the Joint Commission (JC), Occupational Safety and Health Administration (OSHA), California Public Utility Commission and/or any other pertinent federal, state, and local policies relating to the herein mentioned work. All tasks accomplished by the Contractor personnel shall be performed to preclude damage or defacement to patient or Government-owned property, equipment and facilities. The Contractor shall report any damage caused by the Contractor personnel to the COR. The Contractor shall replace items or repair property, equipment or facilities to previous condition, if the Contractor is determined to be at fault for the damage. 2.H. Work Hours: This project shall be completed Monday through Friday, 7:30 AM through 4:00 PM. 2.H.1 The Vendor is not required to provide service on the following National holidays, nor shall the Vendor be paid for these holidays. The following national holidays observed by the Federal Government: New Year s Day 1 January Martin Luther King s Birthday Third Monday in January President s Day Third Monday in February Memorial Day Last Monday in May Independence Day 4 July Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day 11 November Thanksgiving Day 4th Thursday in November Christmas Day 25 December If a holiday falls on Sunday, the following Monday shall be observed as the National Holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a National Holiday by U.S. Government agencies. Also included would be any day specifically declared by the President of the United States. 2.I. Unscheduled/ Emergency Repair Service: 2.I.1. Contractor will be required to provide full-service coverage on the equipment listed. Coverage includes all replacement parts, installation, calibration, labor, and travel necessary to bring the listed equipment to manufacture operating specifications. 2.I.2. Only the CO, COR, or designated alternate has the authority to approve/request an unscheduled service call to the Contractor. 2.I.3. Contractor s technician or technical support shall respond with a phone call to the COR or designee within one (1) hour after receipt of telephoned notification. A qualified technician must respond on-site a maximum of eight business (8) hours after of the initial request for service, unless specified otherwise by the COR. 2.I.4. Equipment shall be restored to full service within 48 hours of arriving on the job site, barring extenuating circumstances. Full service means that defective parts have been replaced with parts that meet or exceed manufacturer's original specifications, meets government s expectations and that the equipment meets or exceeds the manufacturer's original performance specifications. 2.I.5. Technicians responding for unscheduled emergency service must report to the Supervisor, Central Plant Supervisor, or designee (after hours). Upon completion of service/repair the technician shall again report to that person. 2.J. Changes to contract: Only those services specified herein are authorized. Changes to the contract are not authorized and reimbursement shall not be made for any work services performed outside the scope of this contract, unless prior authorization is obtained in writing from the Contracting Officer. 2.K. Safety 2.K.1. All personnel employed by the Contractor in the performance of this contract, or any representative of the Contractor entering on VASDHS property shall comply with applicable Federal, State, and VA safety and fire regulations and codes, which are in effect during the performance period of the contract. 2.K.2. Contractor shall perform work in strict accordance with existing relevant, accredited standards and codes to minimize the possibility of injury to personnel, or damage to Government equipment. Every effort shall be made to safeguard human life and property. 2.K.3. Contractor shall comply with all applicable Federal, State, and local requirements regarding workers health and safety. The requirements include, but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR), Parts 1910 and 1926, NFPA 70E, Article 130.7 29 CFR 1910.132(d)(1)(iii), 29CFR1910.137 2.K.4. Contractor shall provide all necessary tools, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely. 2.K.5. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to himself/herself, his/her employees, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by him/her or his/her employee s fault or negligence. Contractor shall maintain personal liability and property damage insurance prescribed by the laws of the Federal Government. 2.K.6. The VAMC San Diego building 1, main building, contains asbestos. Any worker entering these asbestos containing areas must have an asbestos certification of no less than 16 hours and a valid fit test certification. Certifications must be presented to the VA project manager before any work in an asbestos area may begin. 2.L. Insurance Coverage. 2.L.1. The Contractor agrees to procure and maintain, while the contract is in effect, Workers Compensation and Employee s Public Liability Insurance in accordance with Federal and State of California and/or Nevada laws. The Contractor shall be responsible for all damage to property, which may be done by him, or any employee engaged in the performance of this contract. 2.L.2. The Government shall be held harmless against any or all loss, cost, damage, claim expense or liability whatsoever, because of accident or injury to persons or property of others occurring in the performance of this contract. 2.L.3. Before commencing work under this contract, the Contracting Officer shall require the Contractor to furnish certification from his/her insurance company indicating that the coverage specified by FAR 52.228-5 and per FAR Subpart 28.307-2 has been obtained and that it may not be changed or canceled without guaranteed thirty (30) day notice to the Contracting Officer. 2.L.4. Contractor is required to provide copies of proof of Workers Compensation and Employee Public Liability Insurance within fifteen (15) calendar days after notification of contract award. 3.
