Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document is attached. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2021-05. This solicitation is set-aside for Service-Disabled Veteran-Owned Small Business (SDVOSB) concerns. The associated North American Industrial Classification System (NAICS) code for this procurement is 561720, with a small business size standard of $19.5 Million. The PSC is S201. The VA Network Contracting Office 2 (130 W. Kingsbridge Rd., Bronx, NY 10468) is seeking to procure Temporary Janitorial Services for the James J. Peters VA Medical Center (130 W. Kingsbridge Rd., Bronx, NY 10468). All interested, eligible firms shall provide quotations based on the attached written solicitation. Please read carefully and note the requirements for submission (i.e. 52.212-1 Instructions to Offerors). Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://sam.gov/content/home in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. If paragraph (j) of the provision is applicable, a written submission is required. Offerors must complete the representations regarding certain telecommunications and services or equipment in accordance with FAR 52.204-24. All offerors shall submit the following: SF1449: Complete Block 17a: Offeror Name, Address, POC information, Telephone No., DUNS Complete Block 26: Enter Total Amount of offer Complete Blocks 30a/b/c: Self explanatory Section B.1 (page 4): Enter Contractor information at 1.a. (Entity Name, POC Name-Phone-Email) Section B.3: Provide pricing information; offeror may use the attached excel spreadsheet titled Fillable Price Schedule as a worksheet. When using the excel spreadsheet, only enter unit prices; the remaining information (i.e. Amount) automatically calculates. Carefully review E.1 52.212-1 Instructions to Offerors Commercial Items. At sub-paragraph (b) Submission of offers: paragraph 4: Submit a technical plan and documentation as highlighted. paragraph 6: Enter pricing information at Section B.3 (see page 20), or the excel spreadsheet (Fillable Price Schedule), and submit. paragraph 10: Submit at least three recent and relevant references. Complete the Past Performance Questionnaire and submit with the offer. paragraph 12: Complete the Limitation on Subcontracting Certificate and submit with the offer. All offers shall be sent to the VA Network Contracting Office 2 Point of Contact via email at William.Mills4@va.gov. Hand or physical mail delivery of offers will not be accepted. Submission of your response shall be received not later than June 23, 2021 at 2:00 pm ET at William.Mills4@va.gov. Offerors are advised to be mindful of the size of the email submission(s) and may submit the offer in parts. Offerors are responsible for ensuring the Point of Contact received the email(s). Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f). The award will be made to the response most advantageous to the Government. Responses should contain your best terms and conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" OR ""The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"" Offerors shall list exception(s) and rationale for the exception(s), if any.
