- Combined Synopsis/Solicitation The subject solicitation is amended from Small Business Indian Firm to unrestricted and Due date extended COB June 18, 2021. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation no. 75H71321Q00024 has been issued as a Request For Quotation for a firm fixed price supply contract for brand name or equal dental supplies The performance period includes one (1) 1- year base and four (4) 1-year options. The delivery location and place of acceptance is Chemawa Indian Health Center, Western Oregon Service Unit, 3750 Chemawa Road NE Salem, OR 97305, F.O.B. Destination. The Respondent shall submit a quote and complete Attachment 2-Schedule of Items and attached SF1449. The quote shall include all necessary supervision, management, labor, transportation, equipment, materials, any other direct incidental costs, overhead and profit, unless the cost is identified as a separately priced contract line item. 2. The Respondent shall complete and submit all applicable Representations, Certifications and Other Statements of Offerors. In accordance with FAR 4.1201, representations and certifications shall be submitted to the System for Award Management (www.sam.gov) and updated at least annually to ensure they are kept current, accurate, and complete. 3. If the Respondent identifies an �equal� item in lieu of a brand name item specified, the Respondent shall furnish the required information pursuant to provision 52.211-6 Brand Name or Equal with the quotation, in order to be considered for award. The Government is not responsible for locating or obtaining any information not identified in the quotation. 4. The basis of award is the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Other factors for this solicitation include past performance of the quoter. Past performance information may be based on the Government�s knowledge of and previous experience with the quoter, or other reasonable basis. Award will be made to the responsible respondent pursuant to FAR subpart 9.1. 5. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
