IF YOU ARE INTERESTED IN PROPOSING ON THIS PROJECT, PLEASE SEND AN EMAIL TO jonathan.schrader@ihs.gov ****** Background This patient education resource shall be a patient education system that allows clinicians and staff access to comprehensive patient education for a variety of inpatient and outpatient diagnoses. The patient education resource must provide a collection of health sheets, easy readability levels, and various formats of delivery such as text, phone text, videos, and illustrations. This adheres to PIMCs policy to increase comprehension and participation in the self-management of health care needs. Patient education is an important component of care in both hospital and ambulatory settings.� It is the policy of Phoenix Service Unit�s to ensure that staff will assist the patient and family to gain the knowledge and skills needed to meet the patient�s ongoing health care needs.� Education will be specific to the patient�s relevant health care needs, and will be delivered by methods understandable to the patient and family. Objective The objective of the patient education resource will be able to: Provide easy access to thousands of illustrated patient education handouts from every workstation Provide reliable, researched-based health resources to providers for our patients at PIMC which is web based Provide user-friendly interface that is easy to implement into workflow. Provide on demand and current information to our patients that are ����������� discharged with exit instructions from the Emergency Department or ambulatory clinics. Provide quick and easy access to providers for patient education at the point of care Provide account management features to help standardize education across the organization Automatically update content to reflect changes in practice guidelines Allow providers to personalize education health sheets for the patients.� Brand all patient education sheets with the PIMC logo and personalize printed document with physician name, contact information and patient name. � System Requirements Patient education resource requires a working internet connection as well as current IP address for complete access to resource so that the resource is available from any computer workstation on the PIMC network. Patient Education resource must provide a reliable and working link for access to medical staff that provide patient health information who exit the Emergency Department or ambulatory clinics that includes comprehensive patient information regarding their medical condition. � The Patient resource must be password protected from outside the IHS network and accessible inside the IHS network without a password.� The Patient resource must be compatible with the PIMC EHR so that a link can be placed in the EHR system for provider access.� Contract Awardee Responsibilities & Tasks Provide services for at least 130 beds Provide a patient texting feature Health information documents should be elementary reading level Interface should be easily navigable Diagnoses and health information documents should be easily searched Will create an account for PIMC Will assign a Client Services Manager for the PIMC account Client Service Manager will be available to deliver an initial consultation for the PIMC account admin and a webinar training for staff. Provide usage statistics Provide unlimited access to resources 24/7 via web link Update resources regularly Representative available via phone or email to assist with technical difficulties that may arise. Training The contractor shall provide online training for staff. Web Requirements The contractor shall provide the online link for complete and open access to patient education resource and PIMC will provide IP address range. Period of Performance (POP) 9-1-21 to 8-31-26 Minimum Qualifications Contractor shall have previous experience providing patient education resources to health care providers. Delivery Delivery and installation to all items is required with 2 days after receipt of order or sooner.� Contracting Officer Authority Authority to negotiate changes in the terms, conditions or amounts cited in this contract is reserved for the Contracting Officer. Contracting Officer�s Representative (COR) The person identified below is hereby designated as the Contracting Officer's Technical Representative (COTR) for this contract. (1) Contractors name, address; (2) Contract Number (entire contract number must be included); (3) Invoice number and date; (4) Cost or price; (5) Dates of Service including the number of hours worked; and (6) Remit to address.
