- This is a combined synopsis/solicitation for five (5) EA, Joerns Healthcare, Kit Dolphin 35x93 fluid immersion mattresses to be added to existing mattresses and bed frames at the Spokane, WA Veterans Affairs Medical Center. This is for BRAND NAME ONLY, as prepared in accordance with the format in Subpart 12.6, as supplemented in the attached Request for Quotes (RFQ). A firm-fixed price purchase order is anticipated. Simplified Acquisition Procedures IAW FAR 13 will be used. (ii) The solicitation number is 36C26021Q0662 and is attached. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-04 effective 01/21/2021. (iv) This solicitation is set aside 100% for VetVIP registered SDVOSB authorized sellers. The associated NAICS 339113 code has a small business size standard of 750 employees. The Vendor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, or authorized reseller for the proposed equipment/system such that the OEM warranty and service are provided and maintained by the OEM. All warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. The vendor shall provide new items ONLY; no remanufactured or ""Gray Market"" items. All items must be covered by the manufacturer's warranty. (v) Delivery shall be within 30 days from the time of award and acceptance and shall be delivered to: Veterans Affairs Medical Center 4815 N Assembly St, Bldg. 14 Spokane, WA 99205 USA (vi) Quotes must be emailed to john.macbride2@va.gov and received no later than 11:00 AM MST on 06/24/2021. Quotes may be submitted on the attached RFQ or the vendor s own form. No late quotations will be accepted.
