Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart� 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 246-21-Q-0074 this solicitation is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-5. This is Full and Open Competition, NAICS code 621340; the small business size standard is $8M. The Lawton Indian Hospital requires an Acoustic System Modular Test Booth in accordance with the attached specifications. System shall include installation. The provision at 52.212-1, Instructions to Offerors-Commercial is applicable. The evaluation will be based on technical and past performance.� Must meet all the requirements as stated in the Specifications to be considered technically acceptable.� Submit three references with current phone numbers. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
