- Performance Work Statement Operating Room Video Integration Equipment Storage (continued) 2 Performance Work Statement Operating Room Video Integration Equipment Storage Send a capability statement, no more than one page in length; to include Company Name Duns Number, Socioeconomic Category, brief description of your business entities capability to handle this particular project in the Philadelphia, PA. area, website and GSA Contract number if applicable & contract number/information. Email to David.Santiago2@va.gov with 36C24421Q0845 in the email Subject line, no later than 4:00 pm EST on Wednesday June 23, 2021. I. Scope of Work The Corporal Michael J. Crescenz VA Medical Center (CMC VAMC), located at 3900 Woodland Avenue, Philadelphia, PA 19104, has a requirement for a vendor to provide storage for 23 pallets of electronic equipment. Karl Storz Endoscopy will need storage for 23 pallets of equipment and supplies that must be received by the CMC VAMC but cannot be delivered to the facility until the renovations of each Operating Room Suite is complete. The 23 pallets will be delivered to the warehouse or storage facility all at one time, but due to each Operating Room Suite being completed at different times, the 23 pallets will need to be delivered in various increments to the VA at various times over an approximate 3-year time frame. II. Contractor-Furnished Equipment and Supplies Contractor shall provide all equipment and supplies to store the pallets for the approximate 3-year time frame of all components and supplies for the O.R. Video Integration Project. Contractor shall provide conditioned storage areas that shall protect the warranty of electronic equipment. Contractor shall be fully responsible for the storage of equipment and supplies and shall not hold the VA responsible for any damages or loss due to delivery or storage. Total value of all equipment on the 23 pallets is estimated at $2.5 million dollars. Vendor will need to provide appropriate insurance paperwork to cover this equipment. III. Government-Furnished Equipment The Government will not furnish any equipment for the delivery and storage of the O.R. Video Integration Equipment ordered under 36C244-20-P-0916, purchase order 642-A01065. Contractor shall make all arrangements for the storage, protection and delivery to the VA Medical Center of the items purchased under the above reference order and PO. Vendor / Offeror Requirements. Vendor shall provide storage for approximately 23 pallets, 115 cubic feet each (approximately 2,645 square feet) for a period of up to three (3) years to allow for offsite storage and retrieval of equipment for installation in the CMC VAMC Operating Rooms Vendor requirements include but are not limited to: Locate and confirm suitability of conditioned storage facilities to protect the integrity of purchased equipment. Receive and put away 23 pallets of Karl Storz Video Integration Equipment. Pull, load, and deliver pallets to the CMC VAMC as required by Karl Storz installation crews. Coordinate a minimum of seven (7) deliveries over the approximate 3-year period from the offsite location to the CMC VAMC Operating Room Suite for installation by Karl Storz technicians. Provide monthly storage for remaining pallets until they are called for. Close out storage agreement upon vacating the storage facility. Additional Information. The Contracting Officer s Representative/Service point of contact for this project is Marshall Garrison, Executive Assistant, Patient Care Services, (215) 823-4432. Alternate point of contact is Bethany Purdue, Executive Assistant to the Deputy Director, (215) 823-4389. Equipment warranty shall not start upon date of receipt to the storage location but will commence on the date of installation. The proposed Secure space may be within a warehouse with other tenants. The Secure space proposed shall be securely segregated from that of the other tenants. If there is non-exclusive use of the rental space, CMCVAMC requires floor to ceiling cages to secure its inventory. The Contractor shall provide a valid Certificate of Occupancy for the intended use of the Government and shall meet, maintain, and operate the building in conformance with all applicable current codes and ordinances. The space shall be free of pests, vermin, and insects. The space shall meet applicable egress requirements in National Fire Protection (NFPA) 101, Life Safety Code or an alternative approach or method of achieving a level of safety deemed equivalent and acceptable by the Government. Period of Performance: Period of Performance shall consist of one base year with 2 option years as follows: Base period: August 2, 2021 - August 1, 2022 Option Period 1: August 2, 2022 August 1, 2023 Option Period 2: August 2, 2023 August 1, 2024
