DEPARTMENT OF VETERANS AFFAIRS VETERANS INTEGRATED SERVICE NETWORK (VISN) 16 G.V. SONNY MONTGOMERY VETERANS HEALTHCARE SYSTEM REQUEST FOR INFORMATION 36C25621Q0985 REQUESTED RESPONSES: The intent of this Request for Information is to establish sources in order to define the procurement strategy. Interested contractors are requested to respond in accordance with the following Please respond to this RFI if you are capable of providing the brand name or equivalent products listed in the Performance Work Statement and Schedule found in the background section below. In the response please cite business size status. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing these types of supplies. Please include the following: (a) Proof of OEM or OEM distributor of brand name supplies. (b) examples of prior completed Government contracts, references, and other related information. Responses to this notice are not offers and cannot be accepted by the Department of Veterans Affairs to form a binding contract. Respondents are solely responsible for all expenses associated with responding to this RFI. BACKGROUND: Performance Work Statement Veterans Healthcare System of the Ozarks (VHSO) is requesting five each mobile lifts. VHSO has identified the need for a mobile lift that can extract and insert patients in and out of vehicles and that can also assist with patient six-minute gait evaluation walks. The mobile lifts will significantly reduce the chance of injury to patients and care providers during the transfer and movement of patients. Scope of Work: The Contractor will furnish all labor, transportation, parts, test equipment, tools, and expertise necessary to provide five each mobile lifts to the VHSO. Salient Characteristics: The mobile lift shall be capable of patient extraction and insertion to and from a vehicle, providing support for patient 6-minute gait evaluation walks, picking up a patient off the floor, transferring patients from/to wheelchairs and stretchers, and transferring patients from laying down, sitting, and standing positions. The mobile lift shall be capable of handling patients up to 440 pounds. The mobile lift shall have a pliable lift strap that will extend into the vehicle during vehicle patient extractions and insertions. The mobile lift must come with a universal sling bar 450 with latches. The mobile lift shall be compatible with a variety of loop slings. The mobile lift shall have a low expandable leg base. The mobile lift shall have dual retractable parallel safety arms that allow gait and ambulatory support. The mobile lift shall have wheels with brakes and a turning diameter of 52 inches. The mobile lift shall have a mechanical and electrical emergency lowering device. The mobile lift shall have a lift mast with a control box and hand control. The mobile lift shall have operating controls which are hand controls with 5N of pressure. The mobile lift shall have lifting speed between 27 thru 37 mm per second. The mobile lift shall have a max sound level of 52.8db(A). The mobile lift shall be Lithium ion battery powered with an internal charger. The mobile lift shall have a 24V motor. The mobile lift shall be able to function in the following conditions: Temperature: 50F 104F Humidity: 20% - 80% Air Pressure: 800HPa to 1060HPa Altitude: 2000m max Delivery Schedule: The requested equipment must be delivered to the VHSO by 7/30/21. The Contractor will ensure all deliverables are delivered to the following address and reference the VA s purchase order number as an Attn: line in the shipping address: Veterans Healthcare System of the Ozarks, Attn: Purchase Order #, Warehouse Building 8, 1100 N. College Avenue, Fayetteville, AR 72703. Contractor Requirements: The Contractor shall not perform any service or provide any item that is outside the scope of work without the written approval of the Contracting Officer. Should the Contractor have to come to the VHSO for any reason other than delivery, the Contractor shall be required to log in with VA Police Dispatch (Building 1, first floor) upon arrival to obtain a contractor badge. The Contractor will always visibly wear the badge while on VHSO premises and will be required to turn in the badge before leaving VHSO premises.
