Description
Visual Field Perimetry Machine (Lenexa CBOC location) Statement of Work (SOW) GENERAL INFORMATION The Kansas City VA Eye Clinic Lenexa CBOC location requires the purchase of the following equipment in support of the Kansas City VA Medical Center s mission: One (1) Visual Field Perimetry Machines with printer, table, DICOM, and One (1) FORUM-DICOM Interface License to CZM Instrument (Vx.xx) Scope of Work: The vendor/contractor shall provide the requested equipment to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. Contractor shall provide assistance, in conjunction with the installation by VA biomed of One (1) Visual Field Perimetry Machines with printer, table, DICOM, and One (1) FORUM-DICOM Interface License to CZM Instrument (Vx.xx) and meeting all applicable codes and regulations, drawings, and is subject to the terms and condition of the contract. Background: The Kansas City Lenexa CBOC Eye Clinic requires One (1) Visual Field Perimetry machine with printer, table, DICOM, and One (1) FORUM-DICOM Interface License to CZM Instrument (Vx.xx) This highly specialized equipment is needed for the new location/expansion of the Eye Clinic to the Lenexa CBOC. Type of Contract: Firm-Fixed-Price System mechanical: Visual field perimeter shall be mounted on the automatic wheelchair accessible table. Installation System will be installed by VA BioMed/IT department Maintenance: Systems shall be field serviceable and routinely maintainable by VA Biomedical engineering personnel. Delivery address: TBD by BIOMED and Chief - Optometry SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated in Section A.4 of this SOW. Visual Field Perimetry Machine: Task One: Deliver one (1) visual field perimetry machine with printer, wheelchair table, DICOM, and one (1) FORUM-DICOM Interface License to CZM Instrument (Vx.xx) Deliverable One: Verification of one (1) proper, fully functional and installed visual field perimetry machines with printer. System shall be mounted on the automatic wheelchair accessible table. Task Two: Provide clinical training and configuration on the installed visual field perimetry machine. Deliverable Two: Verification of on-site training for clinical users as specified in agreement. Task Three: Provide full service literature, preferably in electronic format, including as-built drawings and installation data to Kansas City VAMC Biomedical Engineering department. Deliverable Three: Verification of receipt of full-service literature (Operator and Service Manuals), preferably in electronic format, including as-built drawings and installation data. Task Four: Provide the appropriate DICOM and one (1) FORUM-DICOM Interface License to CZM Instrument (Vx.xx) to connect the Visual Field Perimetry Machines to VA Vista Imaging and the Eye Clinic s middleware, pre-existing (Zeiss Forum), an image management system. Deliverable Four: Verification of receipt of DICOM and one (1) FORUM-DICOM Interface License to CZM Instrument (Vx.xx) tocan connect and transfer images to VA Vista Imaging and the Eye Clinic s middleware, pre-existing (Zeiss Forum), an image management system. DESCRIPTION Item Description Quantity Unit Visual Field Perimetry Machine with printer, wheelchair accessible table, DICOM license. Warranty includes 12 months on all products and 24 months on lasers. Training is also included. 1 ea. FORUM-DICOM Interface License to CZM Instrument (Vx.xx) 1 ea. Visual Field Perimetry Manuals Operator and service manuals included 1 ea. SALIENT CHARACTERISTICS All equipment: Must be DICOM compliant as per the DICOM Conformance Statement The image must be captured using a DICOM interface for transfer into VistA Imaging Should be able to connect to KCVA main eye clinic s existing software management system middleware (Zeiss Forum). Technology for Visual Field Perimetry Machine Perimeter should have capability to connect to main KCVA VA Eye Clinic s pre-existing image management software system at a high level such that the image management software can manipulate the raw data derived from the perimeter. Eye Clinic currently owns Zeiss Forum as the image management system/middleware. Visual Field perimetry machine should be a summary measurement of the patient s visual field status, expressed as a percent of normal age-adjusted visual field. Threshold test library needs to include multiple tests including 60-4, 24-2, 30-2, 10-2, and macula. Machine should have a blind spot monitor, gaze and head tracking. Multiple stimulus colors White on White, and red or blue on white. Must perform foveal threshold testing and automatic pupil measurement Should produce results that are directly comparable to perimetry machines that already exist at the KCVA VA Eye Clinic and CBOCs (currently clinics owns 3 Zeiss Humphrey Visual Field Machines). Progression analysis algorithms on the perimeter should utilize evidence-based medicine protocols derived from glaucoma studies such as the Early Manifest Glaucoma Trial. Progression analysis algorithms should be FDA approved to detect progression and not just provide trends. *Contractors providing equal products shall provide descriptive literature describing equal characteristics and must be compatible with current system.
