- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart� 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 246-21-Q-0075 this solicitation is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-5. This is Full and Open Competition, NAICS code 511210; the small business size standard is $41.5 M.� Contractor shall provide a firm, fixed-priced all inclusive quote to provide an Inventory Management System. System shall include a base year plus four option years and one line item for implementation. The provision at 52.212-1, Instructions to Offerors-Commercial is applicable. Evaluation Criteria: The evaluation will be based on technical, past performance and price.� Must meet all the requirements as stated in the Specifications to be considered technically acceptable. The Indian Health Service will not consider your quote for award if you fail to respond to the following Evaluation Criteria. Method and approach- The offeror�s response shall provide conclusive evidence or support their ability to provide services in according to the Statement of Work.��������������������� Best Value- Value of service, material, and offeror�s response to Method and Approach while in compliance with the Scope of Work. Past Experience- Provide contact information and a summary of Inventory Management Services provided to three facilities in the last five years.�������������������������������������������� Registration in the System for Award Management (SAM) database at https://www.sam.gov.�� Please note that all parties interested in doing business with the Indian Health Services must be registered in the SAM database. Copy of FAR 52.212-3 Provisions or Certifications. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
