DEPARTMENT OF VETERANS AFFAIRS VETERANS INTEGRATED SERVICE NETWORK (VISN) 16 G.V. SONNY MONTGOMERY VETERANS HEALTHCARE SYSTEM REQUEST FOR INFORMATION 36C25621Q1230 REQUESTED RESPONSES: The intent of this Request for Information is to establish sources in order to define the procurement strategy. Interested contractors are requested to respond in accordance with the following Please respond to this RFI if you are capable of providing the brand name products listed in the Statement of Work and Schedule found in the background section below. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing these types of supplies. Please include the following: (a) Proof of OEM or OEM distributor of brand name supplies. (b) examples of prior completed Government contracts, references, and other related information. BACKGROUND: Statement of Work Scope Both IM 900 BQ Slit Lamps will be received at CAVHS warehouse, Bldg. 182, at 2200 Fort Roots Drive, North Little Rock, Arkansas. After inspection and internal identification by Biomed Department one will be installed in the North Little Rock VA Eye Clinic at 2200 Fort Roots Drive, North Little Rock, AR 72114 and the other to be installed in the Little Rock VA Eye Clinic at 4300 W. 7th Street, Little Rock, AR 72205. Plan for delivery of equipment on or before 9/1/21. These cameras would enable our staff to document any external suspicious lesions, including possible malignancies for comparison with future exams. This would give good detailed photos of lesions, nevus, adnexa and other external needs not possible with our current internal camera. These cameras would enable us to safely take care of our patients, by documenting these lesions for medical/legal needs. In cases of our ocular plastics, these cameras would enable us to get quality photos for documentation prior to surgery for reimbursement. This camera will attach to one of our current BQ slit lamps. This will aide our high-volume clinic care for these difficult cases without sending them out to community. Contractor shall provide all necessary tools, interfaces, equipment/hardware, software, licenses, delivery, installation, warranty, and training. System Requirements Use of Several Devices, Registration Link The EyeSuite software is designed for optimal patient flow in busy practices. Supporting the Haag-Streit imaging solutions, EyeSuite makes the slit lamp fully networkable. Thus, no proprietary third-party software is required to provide connectivity. If the slit lamp is connected to an EyeSuite server, all the images taken can be accessed remotely from any number of viewing stations connected to the same database. Furthermore, the optional Image Capture Module software enables the import of digital images or documents from other devices (fundus cameras, perimeters etc.) into the HEY patient file. All PII will be stored behind a (MDIA) Medical Device Infrastructure Architect on VLAN Virtual Lan. The C&A requirements do not apply, and a Security Accreditation Package is not required. The operation software includes functions to communicate with the user, to control image acquisition, to store images and data, to process and analyze the acquired images, and to document the examination results. Submittals: N/A Maintenance WARNING! Do not modify this equipment without authorization of the manufacturer. Installation and repairs may only be performed by trained specialists. Contact your Haag-Streit representative for installation, repairs and modification work on the system. The contact details are available at www.haag-streit.com. Only original Haag-Streit replacement parts may be used. 8.1 Device inspection To check the correct function, proceed as follows: Insert the test piece in the radial movement bearing, while aligning the surface at a right-angle to the Microscope Set the slit length to 8 or 14mm Illumination intensity to 50% Set the magnification of the Microscope to max. Set the ocular so that the structure on the test piece is shown in focus. In doing so, turn the ocular from the (+) to the (-) side. Switch on camera For all magnifications, the structure of the test piece in the ocular and in the cam- era image must be sharply imaged Turn on the illumination of the 0.2mm diaphragm Turn on the crosshairs in EyeSuite, the cross must be inside the illuminated dot Servicing To ensure a long service life, the device should be cleaned weekly as described and covered with a dust cover when not in use. We recommend having the device inspected by an authorized service technician annually. Cleaning and disinfection the Haag-Streit slit lamps and their accessories can, if required, be carefully wiped down with ready-for-use disposable 70% ethanol disinfectant wipes. Surface friendly disinfectants (containing aldehyde or aldehyde-free) are also permitted, such as Kohrsolin FF. Training Heidleburg rep will conduct training at time of installation. Delivery & Installation All items will be delivered per VA Policy. To ensure the overall safety of the system, only certified staff of your local Haag-Streit Engineering distributor must install the IM 900 BX WARNING: Only use the supplied USB 3.0 cable (REF 1022373) for the connection to the PC. Only use medically approved PCs or operate via a medically approved isolating transformer. Auxiliary units on the PC (e.g. printer, monitor) must be operated through an isolating transformer. Ethernet may only be used through a galvanic isolation in accordance with EN 60601-1. The power supply unit's mains connector must be accessible in order to allow for disconnection from the electric mains at any time! Installation to be coordinated by POC Johnny Hollis of VA Biomed Department with manufacturer (Haag-Streit) rep. Privacy Officer, Info Security Officer and Records Manager s statements: Information Systems Officer, Information Protection: The contractor will not have access to VA Desktop computers, nor will they have access to online resources belonging to the government while conducting services. If removal of equipment from the VA is required, any memory storage devices, such as hard drives, solid state drives and non-volatile memory units will remain in VA control and will not be removed from VA custody. Privacy Officer: The contractor will not have access to protected Patient Health Information (PHI) nor will they have the capability of accessing patient information during the services provided to the VA and if removal of equipment from the VA is required, any memory storage devices, such as hard drives, solid state drives and non-volatile memory units will remain in VA control and will not be removed from VA custody. All research data available for Contractor analyses is de-identified. Records Manager: Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. The Government Agency owns the rights to all data/records produced as part of this contract. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data.
