- Page 1 of 3 The Department of Veterans Affairs Great Lakes Acquisition Center (GLAC) in North Chicago Il intends to negotiate a sole source contract with MIDWEST FENCE 900 N. KEDZIE Chicago Il. 60651 to fix all the gates: Taylor, Damen, Ogden and Polk so that we can secure them from Police Operations remotely so that we can secure the facility in case of an active threat event for the Jesse Brown VA Hospital campus located at 820 S Damen Ave Chicago Il 60612 This contract will be for a firm fixed price contract. This procurement is being conducted in accordance with FAR 6.302-1, Only One Responsible Source and no other supplies or services will satisfy the agency requirements. The contract will be firm fixed price and the NAICS code is 561621 This Notice of Intent is not a request for competitive proposals. No solicitation documents are available and telephone requests will not be honored. However, if a firm believes it can meet the requirements it must furnish information about its products, services and capabilities as well as references from other customers who are using these products and services provide service for Gate repair and replacement for the Jesse Brown Campus Federal Health Care Center to the contracting officer NLT 11:00AM CST, 25 June 2021. Supporting evidence must be submitted in sufficient detail to demonstrate the ability to comply with the above requirements and attached Statement of Work, to include access to OEM parts for repairs. Responses received will be evaluated; however a determination by the Government to not compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the GLAC will proceed with the sole source negotiation with MIDWEST FENCE Gates Jesse Brown Veterans Affairs Health Care System (JBVAHCS) SCOPE OF WORK Jesse Brown Veterans Administration Medical Center Police Service (JBVAMC) is seeking to fix all the gates: Taylor, Damen, Ogden and Polk so that we can secure them from Police Operations remotely so that we can secure the facility in case of an active threat event. The work is as follows: 1. Polk Street a. Provide and install two linear GSLG-230V 3PH, 1HP slide gate operators b. Provide and install one photo eye c. Provide and install 4 safety reversing edges with peripherals d. Provide and install two sawcut in ground safety loops e. Labor to interface gates with existing card access system f. Test and demonstrate operation 2. 820 Damen Gate a. Provide and install one linear GSLG-120V 1PH, 1HP slide gate operator b. Provide and install one photo eye c. Provide and install one keyed momentary push button d. Provide and install two safety reversing edges with peripherals i. 1 concrete pad/mounting posts to accommodate operator ii. Power to be provided to operator by others iii. Secondary conduit for communication from operator to new key switch box near operator e. Test and demonstrate operation 3. Taylor Street Gate a. Provide and install one linear GSLG-230V 3PH, 1HP slide gate operator b. Provide and install one photo eye c. Provide and install two safety reversing edges with peripherals d. Labor to interface gate with existing card access system e. Test and demonstrate operation 4. Ogden Street Gate a. Provide and install one linear GSLG-230V 3PH, 1HP slide gate operator b. Provide and install one photo eye c. Provide and install two safety reversing edges with peripherals d. Labor to interface gate with existing card access system e. Test and demonstrate operation
