This announcement constitutes the only solicitation; are being requested, and a written solicitation document will not be issued. This solicitation is a ""request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-08. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112, with a business size standard of 1000 Employees. This procurement is being issued as a Brand-Name Only . It is the Government s intent to issue a firm fixed price, single award as a result of this solicitation. The delivery/task order period of performance is 90 days (ARO). Delivery shall be provided no later than 30 days after receipt of order/award of contract. Delivery terms Destination. The contractor shall deliver all line items to James J. Peters (Bronx) VAMC, 130 W Kingsbridge Rd, Bronx, NY 10468-3904. STATEMENT OF WORK Background: The ManoScan esophageal manometry system allows the clinician to map, display and evaluate esophageal motor function. The easy-to-perform procedure precisely quantifies the contractions of the esophagus and its sphincters, providing the clinician with reliable and consistent data for the accurate assessment of GI diseases. The associated benefits of the ManoScan are that clinicians have a more complete picture of esophageal pressure events, more data points and bigger data sets, in addition to displays that are computerized allowing the use of software tools to interrogate different parts of the esophageal pressure profile. At present, Northport VAMC Gastroenterology Service does not have this technology, so patients are sent elsewhere. Acquisition of this equipment will allow Northport VAMC to offer more services on-site without referring the patient to outside Community Care. Scope: Clinical engineering has worked with gastroenterology to evaluate this new requirement to determine that the Medtronic ManoScan esophageal manometry system best meets the VA s needs in delivering a complete manometry hardware and software solution. Specifically, the ManoScan provides a complete physiological mapping of esophageal motor function, from the pharynx to the stomach, with a single placement of a catheter. In addition, this advanced diagnostic technology enables Gastroenterology to better diagnose disease states such as dysphagia, achalasia, and hiatal hernia in our veteran population to support the mission. At present, Northport VAMC Gastroenterology Service does not have this technology, so patients are sent elsewhere. Acquisition of this equipment will allow Northport VAMC to offer more services on-site without referring the patient to outside Community Care. The manufacturer s advanced software acquisition and analytics, including technologies associated with the visualization of values of a physical property detected in an organism over time, and diagnostic system for display of high-resolution physiological data are patented. Clinical engineering and Gastroenterology believe that the ManoScan system represents the best solution for the VA and our veteran population. The Contractor shall deliver and provide all necessary equipment, training and support for one (1) ManoScan Esophageal Manometry System by Covidien Sales LLC, a Medtronic Company. Equipment shall be unpacked and set up by Contractor personnel. Signature of acceptance of delivery by VA POC shall be completed after VA staff have verified that the equipment is fully functional. Period of Performance: 45 Days (ARO) Equipment shall be delivered within 45 days of award and VA POC contacted to arrange installation and training of staff. During the warranty period, the Contractor shall repair or replace without charge, any parts which are deemed to be defective. Contractor shall use OEM parts only. Catheters shall have an extended one (1) year warranty. A three-year service plan shall cover all maintenance and repairs by certified field service engineers. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions**. All offers shall be sent to the Contract Specialist: Wilmen.Joa@va.gov. LIMITATIONS ON SUBCONTRACTING-- MONITORING AND COMPLIANCE (JUN 2011) This solicitation includes clause 52.219-27. Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the contractor to protect proprietary information as required by FAR 9.505-4, obtaining access to proprietary information, paragraph (b). The contractor is required to cooperate fully and make available any records as may be required to enable the CO to assess the contractor's compliance with the limitations on subcontracting or percentage of work performance requirement. This is a combined synopsis/solicitation for Brand-Name Only Stryker System 8 and accessories defined herein.  The government intends to award a contract because of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all bids must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition.""
