- This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number 36C25720Q0829 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. SET-ASIDE: This requirement is a 100% Total Small Business set aside. Please read each section below carefully for the submittals and information required as part of the evaluation. Failure to provide the requested information below shall be considered non-compliant and your quote could be removed from the evaluation process. Offeror quotes shall be evaluated under FAR 13.106-2(b)(3) Comparative Evaluation. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation. The Government is not obligated to determine a competitive range, conduct discussions with all contractors, solicit final revised quotes, and use other techniques associated with FAR Part 15. The contracting techniques associated with FAR Part 15 are not mandatory. The Government will issue a contract to the responsible offeror whose quotation conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The Government shall award a contract resulting from this solicitation. The following factors shall be used to evaluate offers: Vendor is a certified small business. Vendor must be registered is SAM with applicable NAICS code by the close of solicitation. Vendor must provide current industry certification to provide call system installations. Vendor must provide past performance for similar service (CPARS) Technical Approach: The Government will evaluate how the offeror s approach meets the requirements in accordance with the solicitation and Performance Work Statement and indicates a thorough approach and understanding of the requirements. Price: Price will be evaluated independently of the non-price factors. QUOTE SUBMISSION DEADLINE: Quotes must be received no later than Thursday, June 30, 2021 at 10:00 am CST. Statement of Work Customizable Communication Platform West Texas VA Health Care System 300 Veterans Blvd., Big Spring, TX 79720 General Information The West Texas VA Health Care System (WTVAHCS) in Big Spring, TX requires a leased communication board system throughout the Health Care Center (HCC) and Community Based Outpatient Clinics (CBOC)s. There is currently no system in place within WTVAHCS. Contract Title: Customizable online patient health care communication system that utilizes digital signage and interactive screens Background: WTVAHCS requires the establishment of a leased system that is interactive with both the patients and staff. In response to the COVID-19 pandemic, WTVAHCS has been tasked to provide accurate and timely information and education, not only about COVID-19, but also important health and wellbeing messaging. Our facilities will be able to provide Veterans and the community a full range of primary, specialty and chronic health care services in a system that is readily accessible and responsive to change. Scope of Work: The Veterans Experience Office (VEO) and the Public Affairs Office (PAO), underneath the Office of Director, are committed to advancing Veterans health education by providing a contract for a digital patient health education system. The Veterans Experience Office believes that health education content would improve compliance and improve success rates with national initiatives, such as Whole Health and Patient Centered Care. It is imperative that patients continue to manage their health; and it is our duty to remind them and deliver information that helps them do so. Period of Performance: The period of performance (POP) of this contract shall consist of one twelve-month base period. At the discretion of the Contracting Officer (CO), the Government may exercise up to four, twelve-month option periods. Type of Contract: Firm-Fixed Price General Requirements Specific Task #1: Provide licenses as needed and unlimited access to an online patient health care education and health care system that is customizable to support all WTVAHCS facilities. The health care education online patient health and care system should be commercial (the government is not looking to develop a system), and must be capable of providing the most trusted, meaningful and timely communication and education to patients and healthcare staff. Additionally, the online patient health and care system should allow for unlimited capability to have custom messages created for use on waiting room monitors (ST#2). All monitors must be capable of operating independently of the others. Specific Task # 2: Provide and install the following leased supplies in waiting room and exam rooms throughout the WTVAHCS: Quantity Product Description 35 49"" Digital Screen with built in media player, flat wall mount bracket. 35 Cellular Devices 60 22"" digital touch-screen with built in media player, and articulating wall mount bracket. Seeking interactive exam room touchscreen monitors that offers education to Veterans by any healthcare provider through multimedia functions within the interactive touchscreen. This same interactive touchscreen shall be able to display medical articles for managing health issues that can be emailed to Veterans or family members email address, as well as can ask Veterans to conduct patient survey s or access their MyHealthyVet account. This can all be done within the exam room the Veteran is in. 12 of the digital screens and associated cellular devices will be equipped and set up to provide guest WiFi access. The 12 screens devices will be designated by the COR. Specific Task # 3: Provide training on use of the health care education online patient health and care system an onsite consultant (40 hours required during normal working day (8:00 A.M. CDT. to 4:30 P.M. CDT)), and after the health care education online patient health and care system is operational to assist in use and a customer support line that is available 24/7 for general assistance/questions. Specific Task # 4: Provide leasing warranty for the products as is industry standard for scope and duration. Specific Task # 5: Provide combined licensing and support package for the health care education health care education online patient health and care system that will keep the account up to date and the health care education online patient health and care system functioning at 100% of its capability from the beginning from the online patient health and care system s start-up through option period # 4. Specific Task # 6: Provide all labor and hardware to install an independent WIFI online patient health and care system throughout the medical center for the online patient health and care system to run off independently. The monthly service charge for WIFI services shall be included into ST # 5 above. Specific Task #7: C&A requirements do not apply. Security Accreditation Package is not required, IAW with VA Handbook 6500.6, Appendix A, Para. 6. Product Specifications: Commercial-Off-The-Shelf Thirty-five (35) 49"" digital screens with built in media player and flat wall mount bracket Sixty (60) 22"" digital touchscreen with built in media player Twenty-five (125) cellular devices installed at all clinics within the healthcare system Cellular devices shall have strong enough signal to be able to communicate with hardware components located in waiting and exam rooms Capable of operating independently from one another System must be able to display a 3D interactive anatomical model System/monitor that can be drawn on or written on to highlight certain areas of the anatomy System that can display medical articles and email those articles to patient or family member System that allows the patient to conduct survey s or access their MyHealthyVet account from monitor All hardware shall be compatible and interfaceable with the pairing software licenses provided within this requirement The system shall be able to operate its display in two modes: Passive and Active Passive Mode shall feature customizable VA messaging while patients wait in the exam room without touching the device Spanish Guide Access Performance Monitoring: There will be a designated federal employee assigned by the COR to ensure that all equipment is installed and functioning correctly. Travel: The Government anticipates travel under this effort to perform the tasks associated with this contract. Contractor shall include all estimated travel costs in their firm-fixed price line items. These costs will not be directly reimbursed by the Government. Changes to Statement of Work: Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. Security Requirements: N/A Risk Control: If services and products do not conform to contract requirements, the Government may require the contractor to perform the services again in conformity with contract requirements, at no additional cost to the Government, if appropriate. The Contracting Officer may exercise the option by written notice to the Contractor within 60 days. Evaluation of options under FAR 52.217-8 will be accomplished by using the prices offered for the last option period to determine the price for a 6-month option period, which will be added to the base and other option years to arrive at the total price. Washington, DC 20420 REQUIRED REGISTRATION WITH CONTRACTOR PERFORMANCE SYSTEM (CPARS): As prescribed in Federal Acquisition Regulation (FAR) Part 42.15, the Department of Veterans Affairs (VA) evaluates contractor past performance on all contracts that exceed $100,000 and shares those evaluations with other Federal Government contract specialists and procurement officials. The FAR requires that the contractor be provided an opportunity to comment on past performance evaluations prior to each report closing. To fulfill this requirement VA uses an online database, the Contractor Performance System (CPARS). Each contractor whose contract award is estimated to exceed $100,000.00 is required to register. at the following web address: http://www.cpars.gov/ For contracts with a period of one year or less, the contracting officer will perform a single evaluation when the contract is complete. For contracts exceeding one year, the contracting officer will evaluate the contractor s performance annually. Interim reports will be filed each year until the last year of the contract, when the final report will be completed. The report shall be assigned in CPS to the contractor s designated representative for comment. The contractor representative will have thirty days to submit any comments and re-assign the report to the VA contracting officer. The Duncan Hunter National Defense Authorization Act of 2009 (Public Law 110-417) was enacted on October 14, 2008. Section 872 of this Act required the development and maintenance of an information system that contains specific information on the integrity and performance of covered Federal agency contractors and grantees. The Federal Awardees Performance and Integrity Information System (FAPIIS) were developed to address these requirements. FAPIIS is a distinct application that is accessed through the Past Performance Information System (PPIRS) and is available to federal acquisition professionals for their use in award and responsibility determinations. FAPIIS provides users access to integrity and performance information from the FAPIIS reporting module in the Contractor Performance Assessment Reporting System (CPARS), proceedings information from the Central Contractor Registration (CCR) database, and suspension/disbarment information from the Excluded Parties List system (EPLS). SUBMISSION OF QUOTE (FAR 52.212-1 INSTRUCTIONS TO OFFERORS COMMERCIAL ITEMS): The Offeror shall submit their quote on company letterhead and shall include unit price, overall total price, name, address, and telephone number of the offeror, firm's DUNS# and ORCA document in SAM at www.sam.gov. Complete copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and ORCA document. The Offeror shall provide sufficient evidence that they possess adequate resources, capability, experience, responsibility and integrity to meet the technical capabilities to comply with the requirements of the resulting contract. The Offeror shall be registered in SAM.gov prior to contract award. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" OR ""The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"" The offeror shall meet the limitation in subcontracting clause. The subcontract vendor must be similarly situated in the same. The offeror shall outline how they will be in agreeance/compliance with the limitations on subcontracting requirements in 13 CFR §125.6. Any quotes using subcontractors or teaming partners to perform any part of the work must show the subcontractor or teaming partner to be similarly situated as the prime. A similarly situated subcontractors or teaming partners is a company who has the same size standard and NAICS code as the prime offeror/vendor. Similarly situated entity is a subcontractor that has the same small business program status as the prime contractor. This means that: For a HUBZone requirement, a subcontractor that is a qualified HUBZone small business concern; for a small business set-aside, partial set-aside, or reserve a subcontractor that is a small business concern; for a SDVO small business requirement, a subcontractor that is a self-certified SDVO SBC; for an 8(a) requirement, a subcontractor that is an 8(a) certified Program Participant; for a WOSB or EDWOSB contract, a subcontractor that has complied with the requirements of part 127.
