Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this source sought must be in writing. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service- Disabled Veteran Owned Small Businesses and Veteran Owned Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. **Responses to this notice shall include the following on the cover page: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of Large or small business, e.g. Services Disabled Veteran Owned Small Business, Veteran-owned small business (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. (j) If available, please provide any sample pricing available. (k) If available, please provide feedback on the included SOW Statement of Work Employee Assistance Program General: This is a non-personal services contract to provide Employee Assistance Program (EAP) services to employees of VA facilities located in Arizona, California, and New Mexico, which are assigned to the Veterans Integrated Service Network (VISN) 22. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn, is accountable to the Government. Description of Services Definition: The purpose of EAP is to provide appropriate one-on-one professional counseling services for prevention, treatment, and rehabilitation of employees with alcohol, drug abuse, or other biopsychosocial problems that are adversely affecting the employee s job performance and/or conduct. Biopsychosocial problems may include physical, emotional, financial, marital, family, legal, or vocational issues. EAP services may be made available to immediate family members as needed in assisting the employee. The contractor will be expected to provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform EAP services as defined in this Performance Work Statement except for those items specified as government furnished property and services. The contractor shall perform to the standards in this contract. Services will be provided in locations convenient for employees working at and primarily residing near the facility locations identified in technical exhibit 3. There shall be no hidden charges, fees, or billed services to employees for services rendered in accordance with this performance work statement which shall include parking expenses at the contractor s site location. Indian School Rd Phoenix, AZ 85012 Southern Arizona VA Health Care System 3601 S. 6th Ave Tucson, AZ 85723 CALIFORNIA: Greater Los Angeles Health Care System 11301 Wilshire Blvd Loma Linda Health Care System 11201 Benton Street Loma Linda, CA 92357 Long Beach Health Care System 5901 E. 7th St Long Beach, CA 90822 San Diego Health Care System 3350 La Jolla Village Dr San Diego, CA 92161 NEW MEXICO: New Mexico VA Health Care System 1501 San Pedro Dr SE Albuquerque, NM 87108 Areas above also include surrounding geographical areas and Community Based Outpatient Clinics (CBOCs). Background: The VA facilities located in Arizona, California, and New Mexico, which are assigned to the Veterans Integrated Service Network (VISN) 22, provide health care services to Veterans of the U.S. Armed Services. Part of the care provided to Veterans involves providing services in the form of guidance and support to hospital staff. Employees of these facilities are in need of contracted support for employee counseling services. Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this source sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The VA is mandated by Public Law 109-461 to consider a total set-aside for Service-Disabled Veteran Owned Small Business set aside. However, if response by Service-Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined. No sub-contracting opportunity is anticipated. The North American Classification System (NAICS) code for this acquisition is NAICS CodeĀ 624190 Other Individual and Family service ($12M Average Annual Receipts). Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov and complete Online Representations and Certifications Application (ORCA). Additionally, all Service- Disabled Veteran-Owned Businesses or Veteran Owned Small Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. All interested Offerors should submit complete information by e-mail to Nakia.Vance2@va.gov. All information submissions shall be received no later than 4:00 pm Mountain Standard Time on July 12, 2021. After review of the responses to this announcement, the Government intends to proceed with the acquisition and a subsequent solicitation may be published.
