Page 1 of Page 1 of Page 2 of 2 Page 2 of 2 Page 1 of Page 1 of The Veterans Health Administration, Network Contracting Office 02 intends to solicit from a single source IAW FAR 13.106(b) to obtain Therapeutic Horticulture services. Period of performance: 7/15/2021 through 06/30/2022 Basic description of the requirement: The Canandaigua VAMC shall contract with a community organization to provide Therapeutic Horticulture services to eligible beneficiaries of the VISN 2 Network. A bulk of the participants are expected to be within the Finger Lakes Healthcare Network; however, could incorporate upstate New York veterans as well. This model of alternative treatment will offer the services defined below and the contract facility must be located within a 5-county geographic area served by the Canandaigua VAMC, which consists of the counties: Wayne, Seneca, Livingston, Ontario and Monroe. The structure of each program offered shall remain consistent across all Therapeutic Horticultural Sessions. The design for each group shall be to involve small groups of veterans in Therapeutic Horticultural (TH) activities that will enhance quality of life identifiers for participating individuals Place of Performance: Canandaigua VAMC 400 Fort Hill Ave Canandaigua, NY 14424 The NAICS code for this requirement is 624310 Vocational Rehab Services (Size Standard $12 Million). However, interested parties may identify their interest and capability to respond to this requirement by July 7, 2020 at 2:00PM Eastern Standard Time (EST). The Department of Veterans Affairs will NOT be responsible for any costs incurred by interested parties in responding to this notice of intent. Only written responses from responsible sources will be considered. Any prospective contractor must be registered in the System for Award Management (SAM) to be eligible for award. Interested parties are encouraged to furnish information by email only with RESPONSE TO INTENT TO SOLE SOURCE HORTICULTURE in the subject line. All documents submitted, will not be returned. All interested parties shall submit clear and convincing documentation demonstrating their capabilities to satisfy the requirements listed above to Contracting Officer, Janine Childs at Janine.Childs@va.gov. The capability documentation should include general information and technical background describing in detail the prospective contractor's capabilities and experience that will demonstrate the ability to support these requirements.
