Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.� The solicitation number is W912TF-21-Q-0024, and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-05. The associated North American Industrial Classification System (NAICS) code for this procurement is 713940 Fitness and Recreational Sports Centers, a small business size standard of $8.0M.� This requirement is 100% set aside for small businesses, and only qualified vendors may submit quotes. The solicitation will start on the date this solicitation is posted and will end on 7 July 2021 at 2:00 PM Eastern Time.� Please see the attached performance work statement (PWS) for details about the performance locations. The Government contemplates award of a Firm-Fixed Price (FFP) contract. The New Hampshire Air National Guard seeks a contractor who can provide Soldier Welfare and Individual Fitness Training (SWIFT) services as follows: CLIN 0001:� Licensed Dietician (QTY 1= JOB) The contractor shall provide the services of a dietician, licensed by the state of New Hampshire to develop nutrition plans for Soldiers and their families according to the PWS.� The performance period shall be from 23 September 2021 to 22 September 2022. CLIN 0002:� Certified Strength & Conditioning Specialist (QTY 1= JOB) The contractor shall provide the services of a certified strength and conditioning specialist to develop and implement strength training and fitness plans for Soldiers and their families according to the PWS.� Must also be tactical strength and conditioning facilitator (TASC-F) certified as well.� The performance period shall be 23 September 2021 to 22 September 2022. CLIN 0003:� Outreach and Building Community Partners (QTY=1 JOB) The contractor shall develop relationships with local, state, and federal provides to identify external providers who can offer health/nutrition related services to service members and their families according to the PWS.� The performance period shall be 23 September 2021 to 22 September 2022. CLIN 0004:� Marketing (QTY= 1 JOB) The contractor shall conduct a �marketing� campaign throughout the New Hampshire National Guard to ensure service are aware of the availability of these services according to the PWS.� Both digital and print medial shall be used.� The performance period shall be 23 September 2021 to 22 September 2022. CLIN 0005:� Healthy Lifestyle Promotion & Education (QTY= 1 JOB) The contractor shall provide regular training and education on, branch specific, physical fitness and body mass regulations.� Scheduled drill/UTA periods, family events, separate digital/in person programming, meetings upon request, or other venues described in the PWS shall be used to conduct the training sessions.� The performance period shall be 23 September 2021 to 22 September 2022. CLIN 0006:� Reporting (QTY= 1 JOB) The Contractor shall provide reports on the services provided at regular intervals as specified in the PWS.� The performance period shall be 23 September 2021 to 22 September 2022. Telephone inquiries will not be accepted. If the Government responses to technical questions affect the Statement of Work requirements or any other portion of the solicitation, then an Amendment of Solicitation will be issued. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation.
