- Page 2 of 2 The Boise VAMC is seeking Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), or Small Business sources capable of meeting the requirement listed below. The acquisition will be accomplished using commercial item procedures in accordance with FAR Part 12. Minimum Salient Characteristics: Madsen Astera 2 Audiometer Operates in Otosuite Software Integrates with Otosuite Products and Noah Ability to import data to CPRS Fresh Noise Stimulus: Available in entire frequency range (For SF 125-12500 HZ, Accuracy 0.3%) Fully integrated tinnitus questionnaires (THI, THS, TFI,) with ability to store, review and share data during assessment Integrated Pediatric Module with visual reinforcement audiometry (VRA) Pc based two channel audiometer-two separate and identical channels Air, Bone, and Speech audiometer Fully Integrated Quicksin Test Integrated Psychoacoustical Tests for pitch and loudness matching, minimum masking level (mml), and residual inhibition (ri) Special tests such as ten test and sal test Sunshine Control Panel for touchscreen use Product Description MADSEN Astera² is an advanced, 2-channel clinical audiometer that delivers precision and flexibility. The MADSEN Astera² drives efficiency. At its core is OTOsuite software that integrates everything you do into one system. You can transition seamlessly from audiometry to video otoscopy, clinical OAEs, tympanometry and PMM. Store, share and report all data in the OTOsuite software universe or integrate directly with Noah and Electronic Medical Records (EMR). You can operate the MADSEN Astera2 in three different ways: via the optional control panel, a mouse/keyboard connected to a PC, or directly on a touch screen. The Sunshine control panel with large friendly icons enables you to switch from a classical view to a touch screen making daily routine tasks, such as an audiometer test battery even easier. Fully integrated tinnitus questionnaires (THI, THS, TFI,) enable you to store, review and share data during assessment. Psychoacoustic tests are combined into one module for streamlined tinnitus assessment like pitch and loudness matching, Minimum Masking Level (MML), and Residual Inhibition (RI). Integrated pediatric module with Visual Reinforcement Audiometry (VRA) protocol (optional). 1. Contract Title: Madsen Astera 2 audiometer (x2), Zodiac diagnostic tympanometer 2. Background. The Boise VA Medical Center Audiology Department requires two diagnostic audiometers and a tympanometer. 3. Applicable Documents. Handbook 6500.6 Appendix A: Checklist for Information Security in the Initiation Phase of Acquisitions 4. Scope. The vendor will deliver two Madsen Astera audiometers and a Madsen Zodiac diagnostic tool. Vendor must make all reasonable efforts to complete the installation with no disruption of clinical operations in the medical center. Vendor will supply all equipment items and service for a base plus four contract. 5. Specific Tasks. The vendor shall provide the equipment listed in the scope, install the equipment and provide training on the equipment. 6. Performance Monitoring: Not applicable. 7. Security Requirements: The C&A requirements do not apply, and a Security Accreditation Package is not required. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI): N/A 9. Other Pertinent Information or Special Considerations: Only manufactured trained Service Engineers/Technicians or Manufacturer Authorized Service Engineers/Technicians shall perform service/installation and repairs. 10. Risk Control: The Contractor shall obtain all necessary licenses and/or permits required to perform this work. The Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. The Contractor shall be responsible for any injury to any Contractor representative, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by the Contractors fault or negligence. The Contractor shall not discuss any patient issues that were observed during the course of the work. The Contractor shall be escorted by a Veteran Affairs employees and all work shall be observed while performing work at the Boise Veteran Affairs Medical Center. The Contractor shall adhere to all Veteran Affairs policies pertaining to sensitive information and sensitive systems while performing work on all equipment, either remotely or while physically on-site, The Contractor shall not discuss any patient issues that may have been observed during the work. 11. Place of Performance: All work to occur at Boise VAMC, 500 W Fort St, Boise, ID 83702. 12. Period of Performance: The Contractor shall conduct a face-to-face kick-off meeting with the Contracting Officer Representative (COR) and other Government representatives within ten (10) calendar days after award. The work shall be complete within ninety (90) days of contract awards including all training. The North American Industry Classification System (NAICS) is 339112. Any SDVOSB, VOSB, or Small Business firms who wish to identify their interests and capability to provide this product must provide product specifications, performance, and delivery information by notifying the Contract Specialist no later than 13:00 PM Pacific Standard Time, July 09, 2021. Notification shall be e-mailed to Ladell Holmes, at ladell.holmes@va.gov. Any vendor who responds to this Notice must provide credentials to perform the requirement as outlined in the below scope.
