Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is PR47164 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control and Prevention intends to award a firm-fixed-price contract for the following requirement: (Please see attach Line Item Description and Statement of Work).� [OJ(1]�[BPJ(2]�Please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information including a list of line item number(s) and items, quantities, units of measure, and options, if applicable. �Provide your best effort.� Twelve Month Period of Performance: Estimated July 23, 2021 through July 22, 2022. Delivery Address: Outlined in Statement of Work The quote format is at the discretion of the offeror. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website https://www.acquisition.gov/browse/index/far�. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-05 published in the Federal Register on March 10, 2021. � This Full and Open Market Compeition is a Combined Synopsis/Solicitation under North American Industry Classification System (NAICS) code 541511. [OJ(5]�[BPJ(6]�A Firm-Fixed Price purchase order will be issued in writing to the successful offeror. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the System for Award �anagement. To register apply via the Internet at http://www.sam.gov. For questions on registration contact Federal Service Desk at 866-606-8220. Include FAR 52.212-3, Offeror Representations and Certifications with Offer or indicate if it is available online at SAM.gov.� No telephonic quotes will be processed. All responses must be received no later 12:00pm(eastern) on July 16,2021 to Pamela J. Bolling @ pbolling@cdc.gov.� Please send any questions to Pamela J. Bolling by 12:00 pm(est) on July 9, 2021.� After this time period no further questions will be accepted.� .� Award shall be made to the quoter, whose quotation, is evaluated as the lowest priced technically acceptable. The government will evaluate information based on the following evaluation criteria: (1) technical capability factor ""meeting or exceeding the requirement listed in Attachment 1, (2) past performance, and (3) price. The government will evaluate information based on the following evaluation criteria: The following factors and sub-factors will be used to determine technical acceptability: Factor #1-Technical Capability of the Item Offered Subfactor #1 � Ability of Item to Meet Required Specifications � In order to receive a rating of �acceptable� for this subfactor, quote must at a minimum, demonstrate item specifications that meet or exceed the required item specifications. Provide specifications that meet or exceed those provided in Attachment 1. Subfactor #1 � Ability to Meet Delivery Schedule � In order to receive a rating of �acceptable� for this subfactor, quoter shall provide a schedule for delivery that meets the desired delivery date. Factor #2 � Past Performance Subfactor #1 � Recency and relevancy of present/past performance in relation to this effort � In order to receive a rating of �acceptable� for this subfactor, quoter must, at a minimum provide past experience on similar type contract(s). The past experience should indicate successes of the contractor in completing /meeting delivery dates and customer satisfaction. The Government considers �recent� present/past performance to be within the past 3 years. The Government considers �relevant� present/past performance to be present/past performance effort involved in similar scope and magnitude of effort and complexities this solicitation requires. The Government may also use past performance information obtained from other than the sources identified by the quoter. Information obtained from the Past Performance Information Retrieval System (PPIRS) database located at http//www.ppirs.gov is one of the sources that will be utilized. PPIRS functions as the central warehouse for performance assessment reports received from several Federal performance information collection systems and is sponsored by the DOD E-Business Office and administered by the Naval Sea Logistics Center Detachment Portsmouth. NOTE: In accordance with FAR 15.305 (a)(2)(iv)), in the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror shall be determined to have unknown past performance.
