Combined Synopsis-Solicitation for Commercial Items VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Page 3 of 5 Original Date: 10/12/17 Revision 04 Date: 06/10/20 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05. This solicitation is a TOTAL SMALL BUSINESS SET ASIDE Please register with the Department of Veteran Affairs using the information in the form outlined provided in Attachment B. Failure to do so may significantly delay award time and payment due to processing time being 10-14 Business days The associated North American Industrial Classification System (NAICS) code for this procurement is 339114 Dental Equipment and Supply Manufacturing with a small business size standard of 750 Employees The FSC/PSC is 6520 Dental Instruments and supplies The Phoenix VA Health Care System is seeking to purchase Intra Oral Scanners All interested companies shall provide quotations for the following: Supplies Line Item Description Quantity Unit of Measure 1 Intra Oral Scanner (with Pen Grip) 6 EA 2 2 Year Warranty (to cover all scanners) 1 EA 3 Segmentation Laptop 6 EA 4 Segmentation Software (included on all latops) 6 EA Salient Characteristics Scanner Be able to open STL files Have scanner tips which can be autoclaved on average more than 150 times before replacement Have the ability to take a color shade measurement Be able to take intraoral camera HD photograph Be able to do patient specific motion Be able to identify tongue or moving object automatically Operate wirelessly Have trueness/accuracy at no greater than 6.9 +/- 0.9 µm Have precision/consistency of 4.5 +/- 0.9 µm Possess digital impression system without the requirement for powder or impractical stillness in scanning. Have indications for full dentures, aligners and additional ortho treatments. Have Color Pod Wireless 3D Scanners with five autoclavable scan tips Have three batteries and a charging hub. Have an annual operations license and one-year Limited Warranty Allow the design of crown/bridge creation via virtual design with models and splints design exportable via STL Design capability. A gaming laptop is a minimum requirement for having high speed processing of graphic software for viewing and segmenting STL files. The increased speeds are a new technology for helping process 3D imaging software and segmentation files for editing and viewing. Have design and implant software which allows for planning, placement and design of surgical guides via STL Design File Exportation. Have software platform functionality for design, implant, orthodontics, patient monitoring, treatment simulation, smile design and single platform.
