- 36C24621Q0944 This is a combined synopsis/solicitation for commercial supplies prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Quotes are being requested, and a written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ). Submit written quotes referencing RFQ 36C24621Q0944. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05. Any amendments issued to this solicitation will ONLY be available on www.beta.sam.gov The Network Contracting Office (NCO) 6 Regional Procurement Office (RPO) EAST intends to award a firm-fixed price contract for LED Slit Lamp, Tonometer and Accessories at the Salem VAMC, 1970 Roanoke Blvd, Salem VA 24153. The North American Industrial Classification System (NAICS) code for this procurement is 339112 with a business size standard of 1000. This procurement is an open market and small business set aside solicitation to all eligible offerors to submit quotes. All offerors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. The record must be complete and not missing elements such as representations and certifications. The Government will award a contract resulting from this RFQ to the responsible offeror whose quote represents the lowest price technically acceptable for the government. Quotes are due by 10:00AM EST on 16 July 2021. Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. Quotes shall be submitted via email to Jerry.Mann@va.gov. This combined solicitation/synopsis is for the purchase of the following Brand Name or Equal commercial supplies: Manufacture: Haag Streit. Quantity Description 3 Haag-Streit BM900 Slit Lamp with LED Slit Illumination, Basic Model 3 Haag-Steit R900 Tonometerw/2 Prisms 1 Haag-Steit 50/50 Selectable Beamsplitter for BQ Observer 1 Haag-Steit Short Observer Tube with Eyepiece (Requires Beamsplitter) 1 Installation Charge Salient Characteristics Eye Slit Lamp and Applanation Tonometer Eye Slit Lamp Model BM900 Slit lamp must be a convergent, stereoscopic Kepler microscope for short observer. Slit lamp must be 10x and 16x magnifications. Must have a wide range of eye pieces available for advancement as needs change such as 16x and 25x wide angle, length and angle measuring eye pieces. With as set of optional oculars that can extend to 25 and 40x magnification. Must feature the best optical components to show all structures of the human eye in detail. PD and Optical correction are finely adjustable. Slit lamp must have LED illumination with Ergonomic slit illumination control, requires no more bulb changes (lifetime). The illumination system must have 600,000 Lux. Must have a tilting frame that can tilt and rotate up to 8mm in length and width which is critical in good posterior segment observation. Slit lamp must have cross slide mechanism to allow control of both vertical and horizontal movements by a joystick for one hand operation. These movements must be smooth and effortless. All filter and slit controls are situated for ease of use. Tonometer R900 Works with the BM900 Slit lamps. Must be R Style Goldman Tonometer to be compatible with the BM900 Slit lamps. Designed to fit the Haag-Streit 900BM slit lamps. Provides highly accurate Intra-ocular pressure readings during examination with the slit lamp for routine microscopic eye examinations. The tonometer stays permanently mounted to the slit lamp and is swung into place when IOP readings are required. Tonometers must fit Tonosafe disposable Prisms. Already in stock and in use in the Eye Clinic. This procurement is for new OEM medical equipment only for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the government. Evaluation Criteria: The government will award a firm-fixed price purchase order to the vendor whose quotation represents the lowest price while meeting the government s technical acceptability.
