Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart� 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number 75H71221Q00023. This solicitation is issued as a request for quotation (RFQ).� The solicitation document and incorporated provisions and clauses are those in effect through FAR FAC 2021-05 March 10, 2021.� This is set aside to small businesses and the associated NAICS Code is 811219. ��������������� Line item number(s) are as follows: ����������������������� Period of Performance:��������������������������������� QTY� � U/I ����������� Base Year: August 1, 2021 to July 30, 2022������������� ��� 1���� Year ����������� Option Year 1:� August 1, 2022 to July 30, 2023������ ��� 1����� Year ����������� Option Year 2:� August 1, 2023 to July 30, 2024������ ��� 1����� Year ����������� Option Year 3:� August 1, 2024 to July 30, 2025������ ��� 1����� Year ����������� Option Year 4:� August 1, 2025 to July 30, 2026������ ��� 1����� Year The Contractor shall provide qualified personnel to perform a comprehensive on-site visit to each Phoenix Area Service Unit that includes inspection, preventative maintenance, calibration service, and software maintenance of audiology equipment for all service units of Indian Health Service, Phoenix Area Office. There are currently over 40 audiology instruments located within the Phoenix Area Indian Health Service (see Section 1 for complete inventory list). FAR Provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition and any addenda to the provision. FAR Provision 52.212-2, Evaluation-Commercial Items applies and the specific evaluation criteria to be included in paragraph (a) of the provision. �Offeror who does not have SAM registration updated in the SAM.Beta.Gov will be considered non-responsive. Included in this registration is your updated Representations and Certifications.� FAR Clause 52.212-4, Contract Terms and Conditions-Commercial Items nd 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, apply to this acquisition and a statement regarding any addenda to the clause. �Additional Clauses and Terms and Conditions are in the attached.� All quotations are due on July 25, 2021 at 5:00 PM, Phoenix time. The following factors shall be used to evaluate proposals: Technical capability: The offeror shall submit a capabilities statement and include a detailed narrative of how the contractor will accomplish the deliverables. Past Performance Information: The offeror shall provide information concerning recent (within the past three years) and relevant past performance in performing at least three and no more than five contracts of similar size and scope. In addition to the information submitted with the offer, the Contracting Officer shall consider knowledge of and previous experience with the supply or service being acquired, customer surveys and the Government wide Past Performance Information Retrieval System (PPIRS) at www.ppirs.gov or other information. Price: Offerors shall provide prices in the CLIN structure. The Government intends to evaluate quotes and award without negotiations. The initial proposal submitted must contain adequate information to ascertain the contractor�s ability to comply with all requirements of the solicitation and it should contain the offeror�s best terms from a price and technical standpoint. The Government reserves the right to conduct negotiations with one or more offerors, if determined to be necessary by the Contracting Officer. b) Options. IAW the provision at FAR 52.217-5, the Government will evaluate offers for award purposes by adding the total price for all options including the option under FAR 52.217-8, to the total price for the basic requirement. Evaluation of options will not obligate the government to exercise options. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Offerors are advised that the Option to Extend Services Clause at 52.217-8 is included in the solicitation and will be included in the awarded contract. When proposing prices for base and option years, Offerors should consider that the government may choose to exercise the option to extend services at the end of any period of performance (base or option periods), utilizing the rates in effect during the period of performance in which the option is exercised. For purposes of evaluation, the government assumes that any price proposed for base and option periods reflects the possibility that a period of performance may be extended. Offerors shall include a price for the six month option for evaluation purposes only . A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
