- The Ashley National Forest, Vernal Ranger District and the Uintah Interagency Fire Dispatch also known as Vernal Area Facilities has a requirement for janitorial services for buildings owned by the Forest Service to house the Forest Supervisor office staff, Vernal Ranger District office �staff, the Uintah Basin Interagency Fire Dispatch Center staff and the warehouse dispatch, office area �and restrooms. This contract is to provide janitorial services in accordance with the specifications and commercial��practices in a manner that will maintain a satisfactory facility condition and present a clean, neat and professional appearance. Contractor shall furnish all tools, equipment, labor, transportation, and supervision necessary to perform required janitorial services at designated Forest Service buildings in accordance with provisions herein. AT A MINIMUM, OFFEROR SHALL SUBMIT THE FOLLOWING DOCUMENTS BACK WITH THEIR RESPONSE TO THIS SOLICITATION: 1. SF-1449 (Page 1) 2. Schedule of Items (Page(s) 2-3). 3. Exhibit #1 Experience and Equipment Questionnaire (Pages 48-49). 4. Exhibit #2 SF-1413 Subcontractor Form (Page 50) (If Applicable). 5. Technical Response, and Past Performance as Identified in Evaluation Factors for Award. (See Evaluation-Commercial Items Page 61). 6. Section E, Offeror Representations and Certifications, paragraph (b) of provision ��52.212-3. (Offeror Representations and Certifications-Commercial Items), Complete Entire Provision or Paragraph (b) Only, as Applicable. Sign and Date in Margin (Starting on page 64). 7.
